This is a new and exciting opportunity to be involved in supporting and developing our therapy provision to one of our four community hospitals . This is a clinical leadership role which will be based at Newton Abbot Hospital which provides 40 medical beds across two wards.
The post holder will support the therapy team (both Occupational therapists and Physiotherapists), to deliver clinically to people who require further assessment and rehabilitation to support their onward discharge. The post holder will be required to support complex discharge planning and will be integral to the development of both the therapy provision to the ward and an MDT enabling approach to minimise hospital acquired deconditioning through innovative ways.
This post holder will work closely with the hospital MDT members and other clinical leads in the acute hospital and community hospital. The role requires strong leadership to help develop develop a model for community hospital provision for the next 10 years.
We are passionate about reducing the harmful effects of deconditioning and ensure what matters to patients and their families and carers is at the heart of everything we do. We work closely with all other hospital professions and our community partners.
This role will also provide an opportunity to be involved in trust wide professional development projects and provision.
Main duties of the job
The main duties of this job is to make a difference by helping patients through recovery and returning back to health and independence through rehabilitation.
Supporting the wider inpatient therapy leadership team with service provision and staff development
Supporting the development of the Medical beds at Newton Abbot Hospital in conjunction with the Ward manager and other key personnel
Developing the therapeutic delivery model on the medical beds
Facilitating people to leave hospital once they no longer have a criteria to reside
Reducing the use of Pathway 2 options and minimising social care spend on discharge following therapy reviews
Understanding the barriers to timely discharge and finding ways to overcome them
Reducing hospital admissions and Length of Stay
Ensuring appropriateness of community hospital referrals
Promotion and implementation of legal frameworks supporting discharge ensuring multi-disciplinary teams understand the legalities of this framework to be able to work confidently and promote a safe discharge

- To lead a multi-disciplinary team responsible for the delivery and on-going development of therapy to the Newton Abbot Hospital medical beds to support patient's recovery to order to prevent a hospital admission or ensure a timely discharge back to their community and to maximise their independent living to remain in their choice of home
