Are you ready to take your clinical leadership skills to the next level? We're seeking an experienced and dynamic Advanced Clinical Practitioner to join our team and play a pivotal role in delivering exceptional patient care.

As an Advanced Clinical Practitioner with us, you'll have the opportunity to provide dynamic and professional clinical leadership, engaging in advanced clinical activities while upholding the highest standards of care. Working closely with our lead consultant, lead nurse, Deputy Sisters/Charge Nurses, Practice Development Team, and Multidisciplinary Team, you'll collaborate to ensure each patient receives the tailored care they deserve, enhancing their overall experience.

Main duties of the job

  • Uphold Excellence: Ensure top-tier patient care standards, catering to each person's needs for optimal health outcomes.
  • Direct Patient Engagement: Perform in-depth patient evaluations through face-to-face reviews, whether in person or virtually, to gauge needs and potential progression towards further care.
  • Virtual Patient Interactions: Connect with patients via phone or video, providing necessary support and guidance.
  • Attend Daily Virtual Board Meetings: Participate in virtual rounds to stay informed and contribute to patient management discussions.
  • Virtual Ward Prescriptions: Issue prescriptions for virtual ward patients when necessary, ensuring timely and appropriate treatment.
  • Provide Guidance on Patient Care: Advise the virtual ward nursing team on patients experiencing changes in clinical status, facilitating informed decision-making.
  • Facilitate Smooth Transitions: Assist in patient discharge processes, collaborating with primary care and community services, and preparing discharge documentation.
  • Monitor Test Results: Regularly review patient test results to track progress and adjust treatment plans accordingly.

  • Clinical Demonstrate critical understanding of their broadened level of responsibility and autonomy and their own limits of competence and Scope of Practice.

Person Specification

Education and Qualifications

Essential

  • oRegistered Nurse/AHP with current registration
  • oMSc Advanced Practice
  • oManagement/leadership certificate of equivalent or in process.
  • oPost registration experience in clinical speciality or equivalent.
  • oMentorship or teaching qualification


Experience

Essential

  • Competence in the interpretation of diagnostics and clinical decision making
  • Leadership role.
  • Dealing with critical incidents and complaints.
  • Patient care and public involvement.


Desirable

  • Up to date evidence of 4 Pillars of Practice
  • Roster management and safe staffing.
  • Budget management.
  • Experience in leading, managing and developing a team
  • Audit and research in practice


Knowledge

Essential

  • Continuing professional development in portfolio
  • Practice requirements/competencies for RN/AHP in line with mandatory training.
  • Knowledge of trust policy and procedures.
  • Professional leadership within governance framework.
  • National strategy for Nursing and MidwiferyAHP.
  • NMC/HCPC Code
