Early Intervention in Psychosis Team South Warwickshire

Part time 22.5 hours.

We have an opportunity for a band 6 Nurse, Occupational Therapist or Social Worker to join our well established Early Intervention team as a Care Coordinator. We work with individuals who are experiencing the first onset of Psychosis.

Early Intervention in Psychosis is an innovative and evidence-based model. The teams work with service-users aged 14 to 65 who are experiencing first episode or early psychosis, and their families, for up to 3 years, as well as people presenting with an at-risk mental state. There is a strong commitment to provide an excellent standard of care and to continually improve what we do.

The NHS England Access and Waiting Time Standard has led to significant development within the team. We are undertaking specific skills training with the clinicians to support both the clients' recovery and the achievement of these targets.

Main duties of the job

The role involves a detailed initial assessment incorporating the CAARMs assessment to identify at risk mental states (ARMS) or first episode psychosis (FEP). This is incorporated into a formulation to plan the client's care over the next 3 years. The team uses a Recovery philosophy to collaboratively identify the individual's goals and needs and to support them towards achieving these. Many of our clients experience difficulties with housing, finances, vocational activities and social inclusion and the successful applicant will be expected to work creatively, both with other team members and independently to support the client achieve their goals. We aim to support people to independence so they no longer need input from Mental Health Services.

Job description

Job responsibilities

Personal qualities will need to include significant experience in community care coordination and an interest in promoting holistic health and well being, be positive and enthusiastic in what can be a very demanding role, ability to work independently and as part of the team and the ability to structure your own work load and diary utilising a range of interventions tailored to the clients needs. You will need a strong working understanding of CPA and maintain this to a good standard, having strong IT skills. A significant element of the role will be the identification and management of risk, utilising the Steve Morgan tools, including positive risk taking and STORM assessments. You will also be expected to carry out nursing duties as per the job description.

This role requires the successful applicant to be a car driver with access to a vehicle for business use.

Person Specification

QUALIFICATIONS

Essential

  • Relevant Professional Qualification ie. RMN, DipCOT
  • Current unrestricted registration with HCPC/NMC
  • Specialist knowledge across a range of procedures and theory
  • Professional knowledge acquired through degree/ diploma
  • Relevant training and experience specialist short courses
  • Evidence of continuing professional development


EXPERIENCE

Essential

  • Substantial experience in a Community Mental health care setting / Psychosis
  • Developed specialist knowledge gained within a similar environment over a considerable period of time
  • Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
  • Experience of safeguarding and investigations
  • Proven experience of people management

Essential

  • Must have full driving licence and access to a motor vehicle for business use
