Are you a pharmacist ready for a new challenge and committed to enhancing your skills within the primary care setting?

An excellent opportunity for an enthusiastic, adaptable and highly motivated pharmacist has arisen at Swansea Bay University Heath Board, to maximise safe and effective care for patients across primary care.

You will have the opportunity to apply your expert pharmaceutical knowledge and skills to improve patient care through a range of settings including GP practice, virtual wards, care homes etc.

Primary care experience is desirable but we would welcome applications from those with hospital or community experience who are able to clearly demonstrate the aptitude required to work within a busy changing environment.

Main duties of the job

The ideal candidate will be able to demonstrate through experience, their ability to undertake comprehensive patient focussed medication reviews for patients within Primary care. They will need to demonstrate the ability to work autonomously and as part of a wide multi-disciplinary team. The role will involve promoting and supporting the delivery of safe, cost-effective, evidenced based prescribing across Primary Care.

We are looking for applications from enthusiastic, self-motivated clinical pharmacists who are effective communicators with a passion to provide evidenced based patient care.
