The Respiratory Nurse Specialist role is a highly specialised role, designed to provide expert assessment, review, advice, education & medicines optimisation to patients with specific respiratory diseases & home oxygen therapy.

Although previous experience & qualification is desirable, it is not essential as full training & induction to the role will be provided.

The successful applicant will work across the services that make up the Adult Integrated Respiratory Specialist Service (AIRS). The post holder will be required to undertake Respiratory Nurse Specialist inpatient reviews & clinics, Home Oxygen clinics/home visits & support delivery of the Pulmonary Rehabilitation classes independently & in a self-directed manner across Trust & non-Trust sites, which will include undertaking home visits & safety checks in the patient's place of residence and will therefore require a driving license & vehicle to undertake this work.

  • Undertake advanced assessment of patients and effectively interpret and analyse clinical and non-clinical information in a range of complex, long-term diagnoses to develop comprehensive, patient-centred management plans, using the most up to date guidelines/recommendations (e.g., British Thoracic Society, GOLD, GINA), optimisation of current treatment plans/ inhaled/oral therapies, home oxygen assessment & delivery of Pulmonary Rehabilitation.
  • Encourage patients to engage with self-management strategies and services that will positively affect their health and well-being, contributing to reducing mortality, maintaining disease stability and reduce the need to utilise emergency services.
  • Participate in the continuous improvement and development of the Respiratory Specialist Services.
  • Contribute to the reduction in patient length of stay, avoidable harms, and risk of readmission.
  • Support identifying and managing patients reaching end-stage disease, establishing ceilings of treatment, end of life care decisions/discussions, advance care planning conversations.
  • Delivery of training for trust colleagues, peers, and other health-care professionals. Facilitate and participate in training programmes relevant to the specialist service.
  • Provide specialist advice for healthcare professionals working in other clinical areas.
  • Support research studies & actively partake in clinical audit.

  • Respiratory Nurse Service Specific
    • To provide a comprehensive & holistic assessment of patients with Asthma, COPD, and Bronchiectasis who are referred for review as inpatients. Once deemed competent and skilled this will include outpatient within nurse-led and consultant clinics.
    • Demonstrate a capability to engage in decision making in relation to the assessment, planning, intervention, and evaluation of patient care.
    • Provide education and advice for patients with Asthma, COPD, and Bronchiectasis, devising and providing written individualised self-management plans as per national and international guidelines recommend. (Or alternative options if a written plan is inappropriate)
    • Be prepared to undertake assessment and offer advice to patients with Interstitial Lung Disease, where inhaled or nebulised therapy or oxygen therapy are requested by the patients Consultant Physician.
    • Support delivery of the Home Oxygen Service Assessment & Review team (HOS-AR) by ensuring safe and effective assessment and arranging provision of Home Oxygen therapy for the inpatient population. There will be a requirement to undertake oxygen assessment and review in outpatient clinics as part of training & gaining competence in the above.
  • Home Oxygen Service Assessment & Review (HOSAR) Specific
    • Work as an integral member of the Home Oxygen Service- Assessment & Review team (HOSAR) in the outpatient setting both clinics and home assessments. Working in line with the most up to date version of the British Thoracic Society Home Oxygen Therapy Guidelines and disease specific guidelines to provide new and follow up assessments for all adult patients provided with home oxygen across the Fylde coast.
    • Be responsible for organising own workload to meet service and patient priorities, as directed by the Home Oxygen Lead.
    • Maintain the oxygen database to ensure all adult patients in receipt of home oxygen have the necessary follow up appointments booked, and a home safety check completed at the very least on an annual basis.
    • You will ensure that all patients have the necessary consent and an IHORM carried out prior to oxygen installation. When risks are deemed too high you will be responsible for organising MDT meeting with all concerned parties, and complete risk assessments, before installations occur to proactively mitigate risks for the individual patient. Making any necessary referrals to appropriate services required.
    • You must ensure you maintain capillary blood gas sampling competency re CORP/ GUID/461 and follow standard operating procedure Blood gas analysis on OPTI analyser, and interpret results correctly to prescribe, titrate or remove home oxygen. You will ensure that all internal or external quality control errors are reported to Point of Care Team (POCT) and link with the necessary providers to help resolve or raise issues.
    • To be able to effectively explain to patients, carers, and other professionals the rationale for home oxygen therapy and adapt the language used according to receiver.
    • Provide education to patients and families about their illness and oxygen use to improve their quality of life and assist them in the self-management of their condition.
    • Provide the necessary support to the home oxygen patient, their relatives, and carers via telephone to help resolve any issues or provide advice/direct to most appropriate to the services.
    • Maintain awareness of current developments and evidence updates in the field of home oxygen and continually review and develop service policies, procedures, and guidelines in response to this.
    • Work collaboratively with the home oxygen supplier to ensure seamless installation of home oxygen and any ongoing needs of the patients. Raising any concerns to home oxygen lead and/or the home oxygen supplier.
    • Where a Serious Incident Requiring Investigation (SIRI) has occurred, you or a delegated HOS-AR team member must complete the necessary risk assessment and paperwork. The North West home oxygen lead and current oxygen supplier must be informed in a timely manner, and facilitate removal of oxygen if appropriate
    • To have an understanding and work towards meeting the service key performance indicators under the supervision of the home oxygen lead and support in gathering appropriate data to demonstrate the outcomes of the service.
    • To support the home oxygen lead with analysis of data provided from the home oxygen supplier via monthly concordance reports, invoices and transactions ensuring the relevant charges are appropriately applied and use this information to ensure safe provision of equipment for patients and respond to issues or raise queries as required.
  • Pulmonary Rehabilitation Specific
    • Provide expert assessment, review, advice, education & medicines optimisation to patients with specific respiratory diseases.
    • As part of the PR team, you will collaborate with colleagues, to co-ordinate workload distribution. You will be required to practice in a self-directed manner, with confidence and authority.
    • Under the direction of the Team Lead, you will be prepared to support the unqualified PR team members with a specific respiratory disease cohort and all aspects that come under it, within your competency.
    • The post holder will be required to run Pulmonary Rehabilitation classes independently & in a self-directed manner across Trust sites & non-NHS trust sites (e.g., Sports Centres, Community/Church Halls), and will therefore require a driving license & vehicle to undertake this work.
    • Undertake advanced assessment of patients and effectively interpret and analyse clinical and non-clinical information in a range of complex, long-term diagnoses to develop comprehensive, patient-centred management plans.
    • Take active responsibility for an identifiable caseload of patients effectively and efficiently, demonstrating the ability to manage complex cases.
    • Provides support, reassurance and encouragement to patients and their carers as part of the rehabilitation process
    • Be proactive in encouraging patients to engage with self-management strategies and services that will positively affect their health and well-being, empower them to take ownership, contributing to reducing mortality, maintaining disease stability and reduce the need to utilise emergency services.
    • Utilise a wide range of communication skills to ensure compliance with programme this may include negotiation, motivational, non-verbal, or written skills. Often involving conveying complex terminology in lay terms e.g., communication difficulties relating to older population.
    • Using the interdisciplinary assessment tool in conjunction with standard pulmonary assessments act independently to assess and analyse clinical and non-clinical information.
  • Bronchiectasis Physiotherapy Service Specific
    • Support the BPS Specialist Physiotherapist where required to undertake assessment and offer advice to patients with Bronchiectasis, where inhaled or nebulised therapy are the treatment of choice in the outpatient setting if competent & required.

    Person Specification

    Qualifications

    Essential

    • Registered Nurse (1st Level)


    Desirable

    • Independent non-medical prescribing (V300) or ability to undertake the programme
    • Additional specialist training up to Post graduate diploma level or equivalent experience (eg Respiratory disease area)
    • Mentorship, Teaching or Coaching Training/Qualification


    Experience

    Essential

    • Substantial post registration experience
    • Ability to demonstrate application of evidence-based practice
    • Experience of managing and caring for patient with respiratory conditions
    • Evidence of CPD relevant to respiratory


    Desirable

    • Experience in management of complex respiratory patients including assessment, planning and implementation of treatment and care
    • Experience in a leadership and management role
    • Previous experience working within a Respiratory Specialist Service, or ability to demonstrate evidence of equivalent experience
    • Experience of teaching in a variety of settings both formal and informal
    • Ability to demonstrate an understanding and application of audit and research principles
