An exciting opportunity has arisen to join Herefordshire Community Learning Disability Teams as the Band 7 Community Learning Disability Clinical Lead Nurse , Full Time 37.5 hours ( 30 hours will be considered) based at Liverpool Court, Commercial Road, Hereford.
The successful applicant will lead a small but tenacious and committed team who are passionate in ensuring that individuals with a learning disability receive equitable health care alongside managing a clinical caseload and maintaining regular visits for service users and their carers.
This role is challenging, varied and interesting and would include providing clinical leadership and supervision to the team. The team are currently involved in exciting service developments including RcPsych QNLD Accreditation ,STOMP, Dementia pathway, transitions pathways to better meet the needs of individuals with learning disabilities. We welcome creativity, enthusiasm, and passion with a go for it attitude to best meet patients health needs.
You will receive regular supervision, on-going CPD & training, and support within this role to help further develop your skills and competencies in this specialist field.
Main duties of the job
Work within available resources to ensure consistent, safe and timely service delivery including processing and allocation of referrals, access to assessment, day to day coordination of activity.
Provide regular managerial supervision and support to health staff within the CLDT, and clinical supervision as appropriate.
Carry out staff appraisals, in accordance with agreed policy and to identify the learning and development needs of staff, ensuring registration is kept up to date and in order.
Assist the CLDT Manager in ensuring that the service achieves its aims in accordance with the organisation's performance agenda and business plan.
For people on their caseload, be responsible for the whole process of assessment, care planning, monitoring, and review.
Act as a senior/experienced member of the CLDT in relation to risk assessment giving advice and direction as required.
Manage inspections and audit of the team if required.
Provide clinical/management supervision as appropriate to health staff within the CLDT.
Facilitate open communication amongst staff within the CLDT as well as with other teams and services with the aim of promoting multi-disciplinary approaches to care and integrated team working.
