In this role, you'll enjoy the opportunity to broaden your horizons across various teams including CMHT(e), HTT, and Memory Clinic. Take charge of your own caseload while collaborating with a diverse range of professionals from both statutory and voluntary sectors.
Ready to explore your interests? Dive into special areas like supporting carers or working with Younger People with Dementia. As a key player in ensuring clinical governance guidelines are met, you'll also have the chance to contribute to training sessions, educational programs, and engaging in audit and research activities.
Based at Nicholson House in Maidenhead, your days will be spent visiting clients in the local community, making a car essential for your appointments. While most team meetings are currently held virtually, occasional in-person gatherings for meetings, training, or other service-related matters may be required in the future.

Main duties of the job
- Participating as a member of a multi-disciplinary team managing a caseload of patients
- Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
- To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
- Supervising Support Workers and/or students.
- Providing advice to other members of the service and agencies on the needs of service users.
