We are looking for a dynamic, enthusiastic and motivated individual to join our workforce in a busy and well established community team for a fixed-term period of 1 year. Rural North Chichester PCN provides nursing services to an extensive area and has the privilege of working with a diverse population in a rural community. Our community nurses benefit from having continual opportunities to develop an excellent skillset and become autonomous practitioners whilst working as part of a team delivering high quality, holistic care to patients in their own homes.

Primary Care Networks (PCNs) are an innovative service development involving community nursing. The ambition is to integrate multidisciplinary professionals so health and social care can be delivered seamlessly to those who need it. Due to this, community nursing offers a dynamic nursing environment which enables nurses to become confident, skilled practitioners.

Main duties of the job

  • Holistic assessment and care planning for patients in their own homes..
  • Individualised treatment and the promotion of self-care.
  • Admission avoidance and enabling patients to remain at home where possible
  • Cross service collaboration and referral to appropriate member of the multidisciplinary team.
  • Development of extensive nursing skills with excellent training opportunities
  • Professional development as required to maintain area of practice, a negotiated in supervision.
  • An awareness of health, safety and security risk
  • Engagement in service provision and change.
  • Acts as a practice supervisor for staff and students.

Job description

Job responsibilities

Successful applicants will have a recognised qualification in Adult Nursing, a UK driving license and a car. Prior experience in a community setting is not essential as a full induction and training will be provided. We are also able to support newly qualified nurses.

Essential

  • Registered Adult Nurse with NMC registration


Desirable

  • 1 year post reg
  • Experience of community nursing


Knowledge

Essential

  • Good communication and negotiation skills
  • Demonstrates competancy in a range of Clinical Skills
  • Demonstrates the application of research based practice
  • Understanding of the community nurse role
  • Promote independance and choice in undertaking patient care
  • Demonstrates an ability to reflect and learnng from situations


Desirable

  • Experience using computerised patient notes systems


Organisation

Essential

  • Ability to organise workload, to delegate appropriately, take initiative and make decisions
  • Car driver and access to a car for work purposes

  • Ability to work as part of a multi disciplinary team


Numeracy

  • Competant numeracy skills


Clinical Skills

  • Ability to recognise a deteriorating patient

  • Basic IT knowledge
  • Ability to demonstrate a wide range of clinical skills e.g. Wound care, palliative care including syringe drivers, catheterisation, medicines management and administration or willingness to develop skills through training
