This high-profile community-based service covers Mansfield, Ashfield, Newark and Sherwood and receives referrals for patients following an acute stroke.

The service aims to facilitate a smooth and timely transition from hospital to home optimising stroke recovery, function and independence for the initial 6 months post discharge home from hospital.

The team has a strong ethos of patient centred care and high clinical standards. We are looking for an Occupational Therapist that has a keen interest in stroke care and rehabilitation and that is keen for a new challenge and would like opportunities for self and service development.
