We are looking for an enthusiastic, dynamic nurse or therapist with the necessary skills, experience and vision to work within our busy intermediate care team. Based within the South East Essex area, this team works to facilitate hospital discharge and prevent avoidable admissions working with our Virtual Frailty Ward and community and acute providers.

Main duties of the job

We are looking for a Band 6 or Band 7 nurse depending on experience to come and join us.

You will have experience in working within a multidisciplinary environment. You will be able to demonstrate your commitment to effective team working and preferably have experience working in a community setting.

If you have a passion for intermediate care, an interest in patient discharge and admission avoidance, are looking for a challenge we would love to hear from you. The role does include weekend working and following the completion of competencies there is the ability to progress to a Band 7 unless you are applying with the relevant experience to commence directly at a band 7. This could be an excellent development opportunity for the right person and at the gateway of community and secondary care.
