We have an opening for an enthusiastic nurse to join our complex care team at Sandwell. They will be responsible for the clinical and professional management of the team and care packages we provide for.
The post holder will participate in the planning, development and delivery of care of the complex care service and also palliative care. This includes the management of staff and a caseload of patients. The post holder will be flexible and work shift patterns over seven days a week to cover the 24/7 service provision. The post holder will participate in an on call service for families in receipt of complex care provision. To provide improved outcomes for patients by improving the quality of care within Children Palliative and Continuing Care Services, with special focus in complex care.
This is a part time post 22.5 hours per week
Main duties of the job
Develop a referral pathway, which aims to provide a co-ordinated approach for palliative and continuing care children.
To provide support and training to respite carers and nurses within the team.
To liaise with health care professionals in the adult arena of palliative and continuing care to arrange transition of care packages of children up to 18 years of age.
To provide bereavement support and resource information for the staff, family and the child.
Demonstrate advanced practice in holistic assessment, planning, implementation and evaluation of patient care within their own speciality.
To develop appropriate information systems and ensure documentation systems are in place to assist in high-level delivery of care.
Conduct training needs analysis, using the results to develop and deliver a programme which ensures the practice development of those nurses caring for their patients in the speciality.
Overall human resource management responsibility for both continuing care and palliative care teams which includes absence management, staff well being, recruitment and retention activities and performance management in accordance with agreed policies, procedures and protocols. Coordinate nursing activities within the teams, ensuring that there is an appropriate skill mix for the patient caseload within available resources
