An exciting opportunity has arisen to join the Integrated Discharge Team as a Band 6 Discharge Liaison Nurse.

The discharge team play a vital role in the operational flow throughout he Trust. The team are visible through out the Trust and present on every adult inpatient ward and re there to assist when patient no longer meet the Criteria to Reside in the Acute Trust.

We would highly recommend making contact with the team and a shadow shift with the team is possible.

Main duties of the job

The post will be assisting with discharges in adult inpatient areas, you will be involved in complex discharges, supporting the ward MDT with decision making, supporting the Band 3's within the team.

You will be involved in providing advice and support to the wider Trust regarding the different pathway options.

You will be involved in collating information for the regular Criteria to Reside Audit

To assist and facilitate Patient Flow/ wards and patients in a timely discharge process.

To provide and maintain a high standard of evidence based, patient centred care.

To demonstrate good interpersonal skills and to develop practical skills required within the specialty, whist reflecting the philosophy of care.

Training will be provided for all the appropriate areas and processes.

Person Specification



  • Qualified RN


  • Must be able to communicate in English Language, both written and verbally appropriate to the post
  • Teaching/Assessor qualification



  • Knowledge of discharge processes and guidance
  • Working knowledge of Microsoft office suite, in particular Excel


  • Knowledge of Department and Divisional Structure
  • Knowledge or patient flow and impact on OPEL status



  • Evidence of supervisory and mentoring skills


  • Experience of working with multi-agency services
  • Evidence of continuing professional development and to have undertaken post registration training
  • Previous NHS experience
