An exciting opportunity has arisen to join our UCLH@HOME team, a service provided by Whittington Health in partnership with UCLH. We are forefront of integrated care - providing innovative and transformational care that best serves local communities.The post holder will work with colleagues from acute specialities, the emergency department, as well as community services. The role will provide a unique opportunity to be part of a service able to provide a truly integrated care pathway for patients across acute and community settings developing and expanding the services to provide the best care possible for our patients in the best way for them.We welcome applicants from a range of backgrounds including emergency nurses, primary care and acute medicine. Main duties of the job

The post holder will work as part of the UCLH@Home team a service delivered by Whittington Health NHS Trust in partnership with UCLH. The primary responsibility of the Sister/Charge Nurse is to provide nurse led care to patients being treated in a community setting. The nurse will work with the patient, their support workers, their families and the multi professional team to develop a care plan that will meet individual needs and promote optimal clinical outcomes. The nurse will support the patient to manage their health and/or social condition(s) to avoid unnecessary inpatient stay, prevent readmission and unplanned re attendances. The nurse will work closely with colleagues in community services who may also be involved in caring for patients on UCLH@HOME.

The post holder will ensure that all patients are provided with safe and high quality standards of nursing care.

Job responsibilities

Clinical/ Professional Role:a. To provide specialist, high quality, expert nursing care and support for patients who are suitable for UCLH@Home in liaison with the multidisciplinary teams (MDT) through assessment and care planning .b. To complete an holistic review (including advanced physical assessment) of patients c. To initiate agreed range of diagnostic tests within the teams clinical guidelines/care plan d. To refer to other disciplines/professions when required e. To support patients to self-manage, monitor their condition and make informed choices shared decision making f. To act as a named nurse for designated patients within the service g. To ensure that nursing documentation is maintained to required standards and to be a competent user of and participate in the ongoing development of the Trusts planned Electronic Patient Record h. To promote the development of evidenced-based practice i. To participate in clinical supervision to agreed standard j. To ensure high standards of care are given to all patients k. To advise patients on the promotion of health and the prevention of illness l. To be responsible for gaining intravenous access and administration of prescribed medication for patients requiring intravenous medication and for monitoring their care m. To collaborate with medical colleagues in ensuring that appropriate investigations and procedures are performed to complete the patients treatments and that patient and their families are fully informed and understand the purpose for these.

Liaison:a. To promote the service and support clinical staff to facilitate the safe, effective and appropriate transfer of patients identified as suitable for UCLH@Home b. To work with and be responsible for the case management of patients identified for UCLH@Home c. To attend multidisciplinary meetings (MDT) and participate in appropriately cascading information from these meetings to relevant parties involved in the UCLH@Home pathway process d. To record data and support the collection of data for performance monitoring and reports e. To maintain a patient database to monitor patient outcomes and also to audit the effectiveness of the UCLH@Home pathways/guidelines with the aim of developing the service f. To understand the protocols and pathways for UCLH@Home and work with the MDT g. To advise and recommend appropriate discharge options to patient, family and MDT in relation to any further care needs following discharge such as community nursing

  • Current NMC Registration
  • Experience in ED/acute or community

  • Portfolio of varied Clinical experience
  • Knowledge of current issues in the NHS
  • Nurse Prescriber (or willing to work towards)
