Job Details

An exciting opportunity has arisen for an enthusiastic and creative Band 5s Physiotherapist to join North Community Rehabilitation team.

We are looking for a flexible, reliable person who enjoys working as part of a wider multi-disciplinary team.

The community post will cover community rehabilitation patients with a complex and varied caseload, covering a wide variety of conditions including, orthopaedics, MS, MND, Parkinson's, amputees, falls and frailty. Every visit and treatment plan are different, bringing variety and challenge to each working day. The community area covers from Bude to Bodmin over to Launceston in the East. There will also be opportunity to gain some inpatient experience at Stratton Community Hospital.

Main duties of the job

- assessing new patients on our community waiting list in the patient's homes including residential and nursing homes. There may also be some opportunity to bring those patients into a hospital outpatient setting to be seen if appropriate.

- setting treatment goals and following out treatment plans.

- to in reach into community hospital to facilitate early discharge into the community and to continue rehabilitation in the community setting.

- liaising with the wider multidisciplinary team to enable the patient to reach their goals

Person Specification

Education / Qualifications and Relevant Experience


  • Degree or Diploma in Physiotherapy
  • HCPC Registered
  • IELTS unless qualification taken in English
  • Experience of working within a multidisciplinary team and an understanding of roles


  • Experience of working in a health care setting

Knowledge and abilities


  • Knowledge of client centred approach
  • Understanding and knowledge of user involvement issues
