Central and East Community Nurses have recently established a HUB coordination point, where all the Nurses are based within the same building at Whipton Hospital, along with our Social Care, Urgent Care and reablement colleagues, meaning that there is a wide support base at your fingertips.

We are a friendly and supportive team looking for highly motivated RNs who possess strong communication skills to engage patients, families, GPs and the wider multi professional team using a variety of platforms including MyCare, email, telephone and GP patient systems.

We believe that self-development is fundamental to Nursing and we encourage our staff to access further training, be that clinical or academic to further extend your knowledge and skills to be able to enhance the delivery of service to our patients and the community. We are successfully supporting our Nurses to obtain the Specialist Practice Qualification in District Nursing, including enhanced clinical assessment and non-medical prescribing. Several of the team have undertaken university modules of their choice to enhance the service including diabetes, tissue viability and end of life. Career progression within community nursing is supported, with opportunities to develop to a Clinical Nurse Specialist, Nurse Team Manager, Associate Community Matron and Community Matron.

Your role will be pivotal within the local community by making a difference to our patients. Treating patients in their own home can prevent Hospital admission and can assist with early discharge. You will be supported in obtaining these specialist community skills, including complex wound care, administration of medication, syringe pumps, catheter management and many more. As a Community Nurse no day is the same and you will meet patients with varying needs. Lone working is an essential part of the job role. You will be allocated a buddy, have a work mobile phone and have the training and support to ensure that you feel able to undertake this, with daily supervision during patient safety handover.

You will be provided with all the equipment to be able to undertake your job role, including laptop, mobile phone, and all appropriate clinical equipment.
