The opportunity has arisen to work within the Community Older Adults Mental Health Service in beautiful West Wales.

Based in either North Ceredigion or South Ceredigion. Please specify on your application form which area/s you are interested in applying for.

The successful applicant will be responsible for co-ordinating (CTP) a case load as part of a multidisciplinary team.

Main duties of the job

The post holder will be responsible for the assessment of care needs for older adults experiencing mental illness both functional and organic, the development of programmes of care (CTP) and the implementation and evaluation of these programmes of care for a defined clinical caseload.

We are looking for practitioners who are passionate about promoting the well-being and rights of older people, interested in contributing to service development and maintaining a high standard of clinical care.

You will have considerable opportunities for training and professional development and regular clinical supervision will be available.

Full time hours are preferred (but part time hours considered).

The ability to speak Welsh is desirable for this post: English and /or Welsh speakers are equally welcome to apply.

Person Specification

Qualifications and Knowledge

Essential

  • Professional Qualification/Registration from recognised body - ie: RMN, OT, Social Worker
  • Post-basic education in the area of practice to postgraduate diploma level
  • Post registration experience


Desirable

  • Relevant degrees

  • Demonstrates achievements in developments of care standards/procedures guidelines
