The successful candidate will be a Registered Nurse from either Acute & Community or Mental Health background who is keen to develop their assessment skills further in working in an autonomous role within the NHS Long Term Care Service. This role demonstrates the importance of multi-disciplinary working and ensures that the successful candidate has a clear understanding of the policies and procedures associated with Continuing NHS Healthcare & NHS Funded Nursing Care and the associated financial implications as well as the Social Services and Well-being (Wales) Act 2014.

We have invested additional resource into the NHS Long Term Care Service and have implemented a new model of approach around ensuring person centred care. Having entered an exciting period of change we are seeking to recruit a highly motivated and patient focused Long Term Care Specialist Nurses within our Team who will be supporting not only the patients within the care home sector but also the staff as part of a Nursing Home Support Service. Our nurses are also now providing support through a recently developed Long Term Care Pathway which entails supporting secondary care with the Continuing NHS Healthcare and NHS Funded Nursing Care processes. This new pathway is aimed at ensuring a smooth and patient focused transition from acute episode, through discharge planning and then Nursing Home placement.

Main duties of the job

The person we are looking for will be an enthusiastic experienced Nurse who has excellent assessment and communication skills and who enjoys a challenge. We have a team of Long-Term Care Specialist Nurses who work closely with a number of other professionals. The post involves working within the Nursing Home sector, assessing and monitoring individuals as well as providing training and support to Nursing Homes. Furthermore, the successful applicant will be reviewing and assessing patients for Continuing NHS Healthcare and Funded Nursing Care as well as managing MDT meetings relating to funding and discharge planning etc.

The successful applicants will need to demonstrate excellent communication skills and have the ability to work both as part of a team and autonomously. As well as assessing and reviewing individuals to ensure their identified care needs are being met and funding is appropriate, the successful applicant will also monitor standards of care within the Nursing Home setting.

The base can be in either Carmarthen, Llanelli or Pembroke Dock, but may be subject to change. However, you will need to be able to travel across the whole of the Hywel Dda University Health Board area as well as neighbouring counties.

The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

Person Specification

Qualifications and Knowledge

Essential

  • First Level Registered Nurse
  • Current NMC registration
  • Evidence of continued personal and professional development to postgraduate diploma level or equivalent experience


Desirable

  • Commitment to lifelong Learning
  • Qualification in the care of Older Adults


Experience

Essential

  • Experience of working in primary, secondary and/or independent Health Care
  • Knowledge and experience of caring for people who have ongoing care needs
  • Experience in the assessment and care of older adults
