A very exciting opportunity has arisen for a specialist occupational therapist or Physiotherapist to work within the Frail elderly pathway team in urgent care at Airedale General Hospital (acute assessment unit and emergency department) within the acute therapy service. This post is a 12 month Fixed Term contract and the post holder will be required to contribute to the further development and implementation of rapid assessment, planning and discharge pathways at the front door of the hospital. The successful applicant will work closely with community teams, social care and other members of the statutory and volunteer services to enhance flow and discharge through the hospital.

Main duties of the job
- To undertake a supporting role to the Team Leader managing, planning, coordinating, delivering and evaluating the therapy services provided to Frail elderly patients.
- To provide expert training and education for all grades of staff and other health professionals in the therapeutic management of Frail patients.
- To deputise for the Team Leader when requested.
- To apply evidence based, knowledge and skills to the assessment and treatment of patients within the acute service
- To work in partnership with patients, carers, the MDT, and wider statutory, private and voluntary sector services.
- To work with colleagues to ensure the effective functioning of the team.
- To participate in quality improvements to enhance the acute therapy service and the Frail elderly pathway (FEP) therapy team.
- Contribute to student training and development.

- Degree or equivalent pre-registration award in Occupational therapy/Physiotherapy
- State registrations with the Health Care Professions Council
- Completion of course in Professional Practice education
Desirable
- Leadership courses
- Member of relevant Professional special interest groups/organisations (CSP/RCOT)
Experience
Essential
- Relevant and recent experience working at band 5 level in the acute setting.
- Relevant and recent experience working in multiple specialities - This must include at least 12 months on acute medical wards/AAU
- Experience of multidisciplinary team working and working collaboratively with other professions and agencies including statutory, private and voluntary sector services
- Experience of clinical supervision both as supervisee and supervisor
- Recent experience of student training.

- Experience at Band 6 level
