Cardiff and vale UHB community dietetic service has an opportunity for an additional band 6 dietitian to join our busy chronic conditions' management service based at Riverside HC. The role will enable the team to enhance the delivery of the community weight management service alongside as well as our well- established diabetes education programmes. The successful applicant will be responsible for the day to day management of a specialist caseload including weight management and type 2 diabetes.

The post is open to band 6 dietitians with applications also welcomed from band 5 dietitians with relevant experience.Service delivery includes group education, virtual consultations and specialist clinics across the geographical area of Cardiff and Vale. You will have the opportunity to gain further skills through internal rotation within the team in the area of specialist MDT weight management services.

The successful candidate will need to demonstrate excellent communication skills utilising a person-centred MI approach. Weight management and type 2 diabetes experience is an advantage, but training will be provided.

Main duties of the job

Working in the specialist area of chronic condition management dietetics supporting patients for Weight ManagementType 2 diabetes (DM) . Responsible for development, provision and evaluation of the dietetic service for Chronic Conditions specialist area, predominantly weight management and diabetes, including prioritising and managing a highly complex caseload using evidence based client centred principals to assess, plan, implement and evaluate dietetic interventions in the community setting including group and one to one interventions.

The successful candidate will need to demonstrate excellent communication skills, a practical, forward-thinking approach to service development and evaluation and the ability to work in a team.

You are required to travel around Cardiff and Vale as part of this role, including carrying equipment, and some evening and weekend work may be required.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
