Community Mental Health Nurse (Band 6) South West Kent- Older Adults
The successful candidate will need to have relevant skills and expertise to carry out a variety of mental health needs assessments plus be able to plan and deliver interventions, both with individuals, their carers and groups.
You will need to be clinically confident and competent in working with individuals in community settings and be able to work effectively in a multi-professional context.
You should have collaborative and enthusiastic styles as well as have a passion and commitment to develop and promote service provisions for older adults with mental health and dementia related needs.
Main duties of the job
The post requires that you are good communicator and are able to adopt a flexible, adaptable and proactive approach to your work.
As well as providing specialist assessment and treatment within the CMHSOP you will be taking a lead role in memory clinic providing initial assessments, pre and post diagnostic support and treatment for people being diagnosed with dementia.
You will be benefiting from encouragement to continue to develop your own specialist skills, knowledge and competence through participation, reflection, supervision and training and your development will be guided using the trust's appraisal process.

Job responsibilities
The post holder will :-
Plan and provide flexible, client centred care for patients and their carers referred for assessment and support.
Support the deliver of the Clinical Care Pathway work
Take a lead role in the Memory Assessment Service.
Act as care co-ordinator for a designated caseload.
Provide supervision and support for junior staff and students.
Ensure that patients referred to the service are supported before, during and following diagnosis.
Enable patients and their carers to access the care and support that they need.
Contribute to the planning and implementation of therapeutic groups and interventions for clients and carers.
