Are you passionate about improving patient safety? Do you have experience investigating safety incidents, facilitating staff de-brief or After-Action Review and implementing improvements to systems, processes or pathways to prevent incidents? Do you have experience working with carers and families communicating clearly and compassionately and a good understanding of clinical services? Are you able to look at a situation and analyse the multi-factorial contributions of the system.
As an organisation we are demonstrating commitment to providing high quality care for our service users by increasing capacity in our Patient Safety Team with new Patient Safety Investigator Roles. This is a key role within this team in overseeing implementation of PSIRF within the organisation, developing a Just culture as well as implementing new processes and learning responses and is an exciting opportunity to shape our PSIRF implementation journey.
We welcome applications from colleagues considering job share (in order to maintain some clinical contact for example) or secondment, with a minimum requirement of 2 days per week commitment to the PSI role in order to develop skills and competencies. We have funding for 4 posts and are looking for a range of applicants, this post is open to doctors, nurses, midwives and allied health professionals.
Main duties of the job
The remit of the PSI will be to complete thorough Patient Safety Incident Investigations which includes interviewing colleagues, running De-Briefs and working with the Family Liaison Officer to involve patients, family and carers very early in the investigatory process. Excellent report writing skills are essential.
Training and coaching will be a core component to enable colleagues to facilitate their own After-Action Reviews and identify key improvement actions.
Working with other key stakeholders to oversee the implementation of PSIRF implementation and providing updates to the senior leadership team with oversight responsibilities and the ICB. Developing expertise in patient safety, sharing and learning regularly with peers at the regular BOB PSIRF meeting.

2.EDUCATION, QUALIFICATIONS & TRAINING
Essential
- Education to Degree level or similar relevant qualification
- Postgraduate qualification, or equivalent experience in managing complex incidents, complaints and investigations
- Training in SEIPS/Human Factors
- Clinical qualification
Desirable
- Clinical qualification i.e. registered nurse, AHP or doctor or significant experience in Patient Safety in related sector.
- Project Management qualification
- Quality Improvement training to fundamentals level
3.EXPERIENCE
Essential
- Operational experience in the NHS - a minimum of 3 years working in a senior clinical role
- Knowledge of PSIRF and corporate and clinical governance at an operational level
- Experience investigating serious incidents.
- Extensive experience of working in an autonomous fashion, working within defined parameters to meet defined objectives.
- Demonstrable experience of working in complex change environments with multiple stakeholders and securing their engagement
