You need to be patient-centred and able to work with compassion with our patients and their families as they make their transition out of hospital. There is an element of case managing our most complex 'supported' patients through the journey of leaving hospital to ensure a safe and timely discharge.

• Current professional registration
• Relevant post-basic study e.g. specialist pathway
