We are offering permanent roles, but we are open to consider secondments from local NHS health systems in Derbyshire, Nottinghamshire or Leicestershire.

We are in currently developing our integrated hub, with our system partners in line with Discharge to Assess guidance. This is a fantastic opportunity to come and help shape our future processes and work alongside our system partners. We are working with our ward teams to support in discharge plans and complex case management.

Main duties of the job

We are looking for passionate individuals who enjoy working with a range of diverse teams, as we work across our hospital and our community partners. You need to be patient-centred and able to work with compassion with our patients and their families as they make their transition out of hospital. There is an element of case managing our most complex 'supported' patients through the journey of leaving hospital to ensure a safe and timely discharge.

You will be a great communicator and enjoy working with the wider MDT and system partners as part of the integrated hub. You will be driven to support colleagues and advising on discharge plans across QMC and City campus.

Qualifications

Essential

  • Current professional registration
  • Relevant post-basic study e.g. specialist pathway
  • Continuing professional development/ reflective profile


Experience

Essential

  • Relevant clinical experience at band 5
  • Supervision of non registered staff, registered staff and students
  • Evidence of ability to mentor
  • Audit or research within the clinical area


Desirable

  • Demonstrate an involvement in project work
  • Providing staff with feedback on their individual performance and use of the knowledge and skills framework


Communication

Essential

  • Demonstrate effective written and verbal communication skills
  • The ability to communicate effectively with colleagues from different professional backgrounds, and maintaining relationships.
  • A proven ability to coordinate and lead case conferences for complex cases resulting in a satisfactory outcome for all parties.


Desirable

  • Demonstrate evidence of providing formal teaching sessions
  • Demonstration of team working and ability to work within a team
