A great opportunity has arisen for a band 6 Physiotherapist to become part of a dynamic, enthusiastic and friendly team who work with a varied case mix including Rehabilitation, Community and Neurology. The post holder will also be responsible for organising , clinics and domiciliary visits along with admission avoidance visits as a key player within a multidisciplinary team
The successful applicant will proactively manage a caseload of patients with the aim of promoting independence in the community and will be required to liaise with the acute trusts. The ideal candidate will be looking for a challenge, is flexible, a good team player and have varied community / rehabilitation experience, or will have other experience relevant to working in a clinic, ward or community setting.
You will also need to be able to work independently and organise your own work load. The integrated therapy team offers a supportive environment with many opportunities to develop professionally.

Main duties of the job
To provide advanced clinical and professional Occupational Therapy/Physiotherapy to the Community Therapy Team/Community Hospital Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.
Undertake Occupational Therapy/Physiotherapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.
To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care
To provide clinical Occupational Therapy/Physiotherapy skills within the Community Therapy Team/ Community Hospital Team(s), and all other provider organisations.
To be skilled in assessing and treating a range of complex factors relating to the patient's condition, diagnosis and prognosis
To provide specialist intervention tailored to meet the uniqueness of the individual.
i.e. complex manual handling and 24 hour posture management

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
