An exciting opportunity has arisen to work within Swansea Integrated Therapy Team for a full time (37.5hrs) post. Our service remit is centred around admission avoidance to both hospital and long-term care, supporting discharge from hospital through providing reablement within the person's own home and complete assessments in relation to Disabled Facility Grant requests.

This is an exciting opportunity to manager a complex case load of community individuals ranging from paediatrics to older adults.

The successful applicant will be required to travel in a timely manner around the community and participate in a seven day working roster.The applicant should be highly motivated, enthusiastic, flexible and resilient.

The ability to speak Welsh is desirable for this is post English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

o The service is provided to people over the age of 18 years residing within the City & County of Swansea. Assessments and interventions are completed within a wide variety of settings peoples' homes, nursing & residential care facilities, Bonymaen House & Ty Waunarlwydd (step up/down reablement beds) Gorseinon Hospital (step down reablement beds) and Inreach into acute hospital sites (Morriston, Singleton and NPT Hospitals).o The service also accepts referrals for children who may require adaptations to their homes following OT assessment via Disabled Facilities Grants.o Be present at the point of access to Health and Social Care, contributing to the multidisciplinary screening of incoming referrals at the Common Access Point and carrying a caseload of rapid response and urgent enquiries. The team are also an integral part of the Home First MDT screening providing same day response and reablement to people discharged from hospital.o Hold varied caseload and work as part of a multi-disciplinary team to support people to maximize independence. You will carry out functional, holistic, and specialised assessments, and interventionso Interventions may include making recommendations for minor and major adaptations to client's homes.o Supervise junior staff (Health/Local Authority) and support student placements.o Participate in audits and contribute to service development, maintain professional standards and governance.
