The Discharge Suite is a nurse led service that provides patients with the necessary support to return safely to their own home or to move to alternative accommodation which is suitable to their care needs when they are ready to leave the hospital. It is open to referrals from all adult in-patient wards, and it also receives patients from the Acute Medical Unit and the Emergency Department.

The Discharge Suite team is supported by a ward manager and a matron who ensure that all members of the team are fully supported as the days are long and the work challenging. The Discharge Suite sees a huge variety of patients and it gives nurses the chance to enjoy a working environment where you are exposed to lots of different conditions and no two days are the same.

Main duties of the job

To work with all members of the Ward/Department clinical team within the multi-disciplinary framework. Maintain a high standard of patient care under the direction of the Ward/Department Sister/Charge Nurse. To act as a leader in the clinical area, in the absence of a more senior member of staff. To work in accordance with the principles laid down by the Nursing/Midwifery professional bodies and Trust Policies.

Job responsibilities

  • Ensure practice is in accordance with the NMC Code of Professional Conduct and work within limits of competence.
  • To have successfully completed the Trusts preceptorship programme.
  • Maintain a high standard of nursing care applying evidence-based practice.
  • Carry out assessment, planning, delivery and evaluation of nursing care without supervision.
  • Carry out the daily personal care of patients.
  • Maintain accurate records of all care given.
  • Update patient administration systems e.g. PAS to ensure real time data collection and confidentiality.
  • To effectively use information technology for the benefit of patient care in accordance with the Trusts Cyber Code of Conduct.
  • To maintain confidentiality of information (written, verbal, electronic), whilst being aware of the Freedom for Information Act.
  • To promptly and effectively communicate relevant changes in patient status to the multi-disciplinary team.

  • Registered Nurse on the appropriate part of the NMC Register

  • Recent experience of nursing in an acute hospital environment
  • Experience of managing / leading a team and co-ordinating workload
