An opportunity has arisen for a Nurse, Senior Social Worker or Occupational Therapist to join an established team at Fareham Health Centre. Your main responsibilities will include:
-Care coordination of service users
-Response worker function
-Initial assessments and supervision and mentorship of junior staff and students
Consideration will be given for the right candidate to be supported in development to Band 6 competencies.
The post holder will be based at Fareham Health Centre and will need to work flexibly. Due to the rural nature of the role a car driver is essential with access to own car.
The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We are transforming the way we work to provide outstanding care and building truly rewarding careers.
The Fareham Community Mental Health Team operates works closely with the Gosport, Havant and Petersfield and Borden CMHT and some cross cover maybe required.
We support and encourage our staff to developing their clinical skills as well as attending post graduate training such as physical health and nurse prescribing.

Main duties of the job
To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.
This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
Role further includes acting as clinical and case management supervisor and mentor.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
