We are looking for a Senior Mental Health Practitioner to work in our Home Treatment Team based at North Devon District Hospital. The successful candidate will be working on a shift based rota (including weekends). You will play an essential role in providing 24/7 (Excluding night Shifts) specialist mental health services to those who need them.

Our Home Treatment Team (HTT) is a community based service who support people experiencing severe mental health issues and who require 'crisis' support. The aim of the team is to help people to manage and resolve their crisis through assessment and treatment in their home environment. Our Home Treatment Team also supports individuals who are being discharged from hospital, enabling them to continue their recovery at home.

Main duties of the job

The successful candidate will become part of a multi disciplinary team carrying out Home Treatment Team assessments, creating collaborative care plans that includes families/carers, and signposting to appropriate agencies.

The role includes assessing the suitability of home treatment for individuals presenting with acute mental illness or severe distress, which can be safely managed in the person's home.

It will be your job to devise interventions that will help people to recover. You will play a key role in a dynamic multidisciplinary team, working with multiple agencies on a daily basis.

You will be confident working without direct supervision with the knowledge to devise individual recovery plans for the people you see. Its important you can communicate effectively, not only with your colleagues but the people you care for and their families and carers. You will be at the center of their care.

The post holder is required to hold a UK driving license to carry out home visits in pool cars.To apply you need to be an experienced mental health professional with a professional mental health qualification.

Job responsibilities

When working in the HTT team, practitioners undertake assessments and treatment as required. The person will lead in the assessment, co-ordination, planning, implementation, evaluation and delivery of high quality home treatment for people in acute Mental Health crisis. With the aim of promoting their recovery and maintaining them in a community setting rather than in hospital.

As a band 6 practitioner you will operate within a community setting and work within an multi disciplinary team which aims to reduce hospital admissions and facilitate early discharge through home based treatment. You will also work closely with inpatient and community services.

Holding a full UK driving license is essential to the role to undertake visits within home treatment team pool cars.

Person Specification



  • -A professional mental health qualification eg: RMN, CQSW / ASW, BSc/Diploma in OT
  • -A current registration with a professional body ie NMC.



  • -Substantial experience of managing a busy caseload of clients with a range of mental health problems.
  • -Experience of working in a multi-disciplinary and multi-agency environment.
  • -Providing clinical supervision.
  • -Assessment experience, Particularly mental health and managing risk.
