Contract Type
Full Time
An opportunity has arisen for a Senior Social Worker or Occupational Therapist to join our team.

Your responsibilities will include care coordination of service users, response worker function, physical health monitoring and supervision and mentorship of junior staff and students.

The Bordon and Petersfield Community Mental Health Team operates between two sites, the main team base being Petersfield Hospital with a smaller base at Chase Hospital in Bordon and some working in the Havant Area. The team is well known for being approachable and open. We support each other and have developed a culture of honesty and integrity where problems are resolved quickly.

The post holder will be based at Petersfield Community Hospital and will need to work flexibly. You will need to establish good working relationships with the local GPs, Italk service and other community/voluntary organisations. We're investing in our mental health services in South East Hampshire, transforming the way we work to provide outstanding care and building truly rewarding careers.

We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.

Due to the rural nature of Petersfield and Bordon a car driver is essential with access to own car.

We are interested to hear from you and what new ideas you can bring. For further information or to arrange an informal visit please contact Emma Turnbull.

Main duties of the job

  • To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.
  • This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
  • Role further includes acting as clinical and case management supervisor and mentor.
  • To make links with the Primary Health Care teams.

About us

Southern Health is one of the largest NHS Foundation Trusts in the UK, specialising in mental health and learning disabilities, as well as offering physical health community-based services.

As a Trust, our footprint spans more than 200 sites across Hampshire, wholeheartedly committed to providing the best possible healthcare, reaching a range of diverse communities.

- We function as a unified team, placing patients and staff at the forefront of all our endeavours.

- Our 7,000+ workforce is skilled and diverse, with a deep commitment to ongoing staff development. We offer customised training, leadership pathways, and a plethora of career opportunities.

- We respect and listen to your thoughts, ideas, and concerns via well-established network of staff groups to increase awareness and appreciation of equality, diversity, and inclusion.

Building on all this, we are currently working closely with other local NHS Trusts to integrate all our collective community, mental health, and learning disability services across Hampshire and Isle of Wight. Our new organisation is set to launch in April 2024 and will incorporate Southern Health, Solent, Isle of Wight and Sussex Partnership Trusts.

While the new Trust will be substantially bigger, it will continue to cater to the unique needs of different communities, streamlining services and making healthcare across the county more accessible.

Join us as we embark on this exciting journey to shape the future of healthcare in Hampshire.
