Are you an experienced Staff Nurse looking for a change in direction? Are you enthusiastic about safe discharge planning for your patients?

The Complex Discharge Team is looking for a dynamic, innovative and enthusiastic experienced nurse to join the teams, to ensure the safe, effective, appropriate and timely discharge of patients from hospital. As a registered nurse you will have significant experience in an acute hospital care setting, with extensive knowledge of the discharge process through relevant post-qualification training and experience.

As the Band 6 within the team you would be supporting the band 7 and band 6 Complex Discharge Nurses to ensure the timely completion of complex discharge assessments and service provision and have an awareness of complex discharge including, but not limited to continuing health care processes including End of life, Mental capacity, DOLS and safeguarding and Discharge to assess. You will be a self-motivated team player, who will uphold and promote our Trust values. You will need to demonstrate excellent communication skills and be able to confidently liaise with all members of the multi-disciplinary teams and external agencies involved in the discharge process and provide support and teaching to the wards, attend MDT and lead and signpost on all complex discharge issues in the trust.

Main duties of the job

To participate in effective and timely transfer of patients from the acute Trust through identified social and health care pathways, in order to improve the coordination and delivery of care thereby reducing delays and expediting discharges.

To support the coordination of discharge arrangements for patient with complex needs as part of the National Hospital Discharge service, ensuring that the patient is at the centre of all discharge planning.

To ensure patients are discharged at their 'fit for discharge' date in an efficient and timely manner.

To ensure that patients are discharged to their place of usual residence: otherwise they are placed with services relevant to their needs, that opportunities of independence and self-management are maximised and that reliance on long term care is mitigated wherever possible thus reducing the likelihood for readmission

You should be confident in completing holistic, nursing needs based assessments as you will be and will also need to be confident liaising with all MDT, patients and their families. This job role includes daily liaison with all clinical ward areas, including emergency department as needed and excellent communication skills are essential for this post.

You will also support the Clinical lead Matron and Band 7 in developing the complex discharge service, with processes, governance and staffing.

Person Specification



  • *Registered Nurse on the NMC register.
  • *Maintained relevant continuing professional development in the past 3 years which supports revalidation.



  • *Ability to act on own initiative with evidence of sound decision making abilities
  • *Ability to demonstrate up to date experience and depth of knowledge and clinical competence
  • *Experience of MDT working


  • *Experience in an acute hospital/community setting.
  • *Experience of caseload management
