An opportunity has arisen, for a passionate and enthusiastic individual to be part of our innovative and crucial service in Wisbech. We welcome applications from newly qualified Mental health Nurses, Mental Health Occupational Therapists and Social Workers. You may be looking to move on from your first qualified post to a community role.

We are looking for people who have a relevant NMC or HCPC registration and:
  • Relevant pre-registration experience of working in mental health environments, however this is not essential.
  • Previous experience and skills which can transfer into this role - good communication, organisational skills and timekeeping amongst others.
  • We welcome applications from people with lived-experience of mental health challenges.
  • People who are passionate about working with older people in the community experiencing mental health challenges.


Main duties of the job

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

The role involves (but is not limited to):
  • Providing support interventions for people with significant mental health conditions.
  • Working within a multi-disciplinary team with colleagues in the Mental health and Neighbourhood Teams.
  • Monitoring and reviewing the use of prescribed medications.

The successful candidate will be required to be flexible but primarily based at North Cambridgeshire Hospital, Wisbech. We will provide the support to ensure your development within your role. Job responsibilities

  • To provide an ongoing assessment of the care needs, plan care in collaboration with the service user, their carers (where appropriate) and MDT colleagues. Delivery care in a safe and effective manner and ensure that needs are reviewed on a timely basis commensurate with service user needs.
  • Supporting service user medication concordance in association with MDT colleagues. This will be achieved by demonstrating a working knowledge of medications and policies commonly used in older peoples mental health services including relevant NICE guidance e.g., anti-psychotic prescribing in dementia care
  • To actively engage in partnership working in respect of the patient, carers and service development
  • To have a working knowledge of current mental health and wider legislation, in addition ensuring effective application of CPA assessment and care planning principles and effective risk assessment.
  • Contribute to the assessment, treatment, evaluation, and effectiveness of treatment / care offered and review plan with multi -professional team / team manager.
  • Liaise effectively with other members of the service team, primary health care teams, residential/nursing homes, adult social care and other organisations contingent upon the client.
  • Employ specific skills to help resolve/manage problems in older persons mental health e.g. anxiety management strategies, behavioural management strategies in dementia, family support skills etc.
  • Be familiar with and observe statutory and local policies in the prescribing, custody and administration of medications in line with the Trusts medicine management policies and procedures.
  • Maintain a high standard of written reports, records of visits/interventions, care plans recording the progress of clients in line with standards for records and record keeping
  • Participate in:a. Clinical case management and supervision groups.b. Lead as appropriate, clinical meetings/reviews.c. Administrative/management meetings as required.
  • To be familiar with up-to-date knowledge of current trends, pertinent research findings within this specialised area of practice. To keep up to date with nursing practices ensuring that PREP requirements are met.
  • To engage in clinical supervision and mentorship to enhance professional development


Person Specification

Education / Qualifications

Essential

  • RMN level 1 RMN level 2 . or HCPC registered professional with mental health training i.e. Occupational Therapist or Social worker


Experience

Essential

  • Experience in acute or community mental health setting - can be part of your training.
  • Experience of working with people with a diagnosis of dementia - can be part of training.


Desirable

  • Community experience
  • Primary care experience


Knowledge & Skills

Essential

  • Good communication skills
  • Ability to work flexibly as part of a team
  • Accepts responsibility and accountability for own work and can define the responsibilities of others.
  • Standard keyboard skills and ability to communicate through IT using packages such as Word
