The post holder will work across North Yorkshire within the TB Nursing Service section of the Infection Prevention and Control (IPC) Team. Working alongside the TB Specialist Nurse / service lead and Admin Support Worker, the post holder will deliver a high quality service working autonomously to co-ordinate and deliver comprehensive care to patients (both adults and paediatrics) across North Yorkshire.

Main duties of the job

  • To receive and use highly complex and sensitive information to provide specialist, patient-centred nursing advice to those infected with and affected by Tuberculosis (patients, their families and contacts as well as health care workers)
  • To manage a case load and provide standard / enhanced case management to patients (both adults and paediatrics) affected by active TB disease or latent infection.
  • To administer / refer for TB screening tests and BCG vaccination as appropriate in accordance to protocol/guidance.
  • To establish effective partnerships with patients and their families and acts as patient advocate to ensure their needs are met appropriately.
  • To work (mostly alone) from a variety of locations across North Yorkshire including patient's own homes, clinics, universities and schools, assessing hazards and risks and implementing strategies to minimise these.
  • To support patients in adhering to difficult treatment regimes by assessing and developing strategies to overcome barriers to adherence.
  • To act as a role model and resource for other professional groups and students by providing excellent care to TB patients
  • To provide individualised patient care based upon the principles of evidence based practice
  • To recognise, investigate and take the necessary action to resolve problems in relation to failing treatment
  • To act as advocate, as required, on behalf of the patient in a variety of settings to ensure their needs are met appropriately
