Job Details

The Rotherham NHS Foundation Trust is an Integrated Acute and Community Trust which is committed to partnership working across Health and Care, built on a strong home first ethos. Working as part of a multi-disciplinary Health and Care team the role holder will be part of our multi-disciplinary Urgent Community Care team. They will provide high quality senior Nursing support for people requiring an urgent community response Out of Hours, enabling them to be cared for in their own home thereby reducing avoidable admissions and supporting timely discharge from the acute and community bed setting.

Main duties of the job

Provide effective triage and assessment on a rota basis ensuring Patients receive the right level of care, at the right time and in the right place according to the needs of the individual and within available health and social care capacity.Provide skilled Nursing advice, support and care to Patients, their family/supporters and carers.Have continuing responsibility for overall management of a Patients and caseloads.Provide strong clinical leadership to the urgent Out of Hours Nursing team.Promote good team working and an environment to enhance job satisfaction and ensure team members feel valued and empowered.

  • Experience of Community Nursing
  • Experience of Clinical Triage
  • Experience of Team Management
  • Experience of the development of individualised care plans and associated process
  • Experience of working in a multi-disciplinary team
  • Demonstrates personal and professional maturity
  • Demonstrates responsibility and an awareness of service user needs
  • Mentoring student Nurses
  • Demonstrates the ability to deal with difficult and challenging situations



  • Demonstrate a clear understanding of community nursing, primary and social care
  • Policies, practices and procedures e.g. NHS national policies, standards, requirements and directions that relate to measuring and improving the quality and safety of patient care
  • Understanding of community and multi-disciplinary working and inter-professional relationships
  • Current evidence based practice and research related to nursing and health care in the community.
  • Knowledge of recent developments in Health and Social Care


  • Some experience working in a community setting having had exposure of chronic conditions and acutely unwell patients.
  • Knowledge of Long Term Conditions or willingness to complete appropriates learning modules.
