We have an exciting opportunity for the right person to join our team and play a major role in the leadership team leading the Pershore & Upton, Malvern and Evening & Nights Community Nursing Neighbourhood Teams.

The role entails the operational management of these 3 Teams which includes the line management of six Band 7 direct reports and Band 4 PA budget management Quality and performance monitoring and service improvements.

The post holder will need to take part in system calls regarding patient flow and support with the escalation of delays where needed.

Leading and monitoring Innovations around End of Life, Virtual Wards, OT pilot, ACPs, intensive support and catheters are also key work-streams.

This role includes being part of the on-call rota for the Trust.

Main duties of the job

You will work closely with the Deputy Associate Director and fellow Integrated Community Service Leads as part of our Senior Management Team.

Accountable to the Deputy Associate Director you will support strategic leadership to clinical services operationally on a day-to-day basis and will be aligned with the following services: Neighbourhood Teams, MSK / APP Services, and our Housing, Adaptations and Equipment Services.

The post holder will be involved in an extremely exciting and challenging period of service development with the further roll out of the Ageing Well Agenda, enhanced models of frailty care and developing closer working and operational alignment with partner organisations. You will require energy and a commitment to quality improvement across this diverse portfolio.

Job description

Job responsibilities

The successful applicant will work within the beautiful location of Malvern, Pershore and Upton, and join an experienced supportive team of Service Leads. The role is full time, with on-call management duties to cover weekend and the out of office period during the week. A professional health qualification and up to date registration is a requirement.

As a Service Lead you will support the realisation to build a fully integrated community offer for our patients in line with the Fuller Report. We work closely with General Practice and other system partners to:
  • blurthe lines across local health and care organisations - integrating management and leadership to enable a more 'joined up' operational solution - ultimately benefitting our patients.
  • optimise the potential to jointly implement the Fuller recommendations across our PCN footprints - focussing on optimising joint resources across the PCN footprint (rather than working within organisational silos).
  • secure effective ways to unlock potential efficiencies in relation to holistic care provision across organisations - with the patient at the heart of everything we do.
  • improve patient experience and ultimately result in a more effective, efficient offer (byjointly delivering outcomes,reducing visits across organisations, and supporting the green strategy).
  • optimise local assets (via District Collaborative engagement) to promote local self-help solutions.

Qualifications

Essential

  • A current professional qualification (RMN, RGN, OT, Physiotherapist)
  • Registered to practice in the UK.
  • First line management qualification or equivalent experience
  • First degree or equivalent in health care subject
  • Evidence of on-going CPD
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer


Desirable

  • Masters Level Qualification or working towards


Experience

Essential

  • Significant post graduate leadership experience in core areas
  • Proven record of implementing change and developing services
  • Experience of managing a budget

  • Developing policy/strategy


Knowledge

Essential

  • Good working knowledge of health and local authority services
  • Understanding of the corporate agenda of the Trust and able to communicate key issues across the locality.
