An exciting opportunity has arisen for a talented, visionary individual with widespread experience in the field of Patient Safety. We are looking for an experienced and innovative leader who will support the Patient Safety Specialist and wider Patient Safety Leadership Team in their ambitions to implement the Alder Hey Patient Safety Strategy and PSIRF. The postholder will have proven leadership skills, experience of working at a strategic level, be highly visible, and be able to motivate and engage individuals and teams across the organisation. With demonstrable and extensive investigation and learning response expertise, the successful applicant will hold a well-developed skillset to help advance the delivery of the Trust-wide Patient Safety Programme.

Main duties of the job

The post holder will :
  • be expert in providing robust and evidence-based learning reviews utilising agreed frameworks which support achievable patient safety improvements and learning.
  • work to achieve the Trust's objective of a psychologically safe learning environment using restorative, just and learning principles.
  • engage with patients, families, carers and significant others to ensure their voices are heard and findings and learning/ actions are agreed prior to completion.
  • work collaboratively with subject experts to ensure efficacy of reviews.
  • ensure all staff involved in the incident are given the opportunity to participate, review drafts and agree the final learning review prior to quality assurance processes and final sign off by Executive leads.
  • work to the patient safety PSII and learning review criteria, standards and timescales set out within the Patient Safety Incident Response Framework.
  • signpost those who need further support post incident appropriately including families, carers, significant others and staff.
  • escalate any emerging and urgent clinical risk appropriately.
  • extrapolate and escalate any conduct or professional concern directly with the Patient safety leads at the earliest opportunity.
  • attend and be available, as is reasonable, to present the findings of reviews at internal and external forums as requested.
  • ensure their work aligns to the relevant polices within the Trust.

Job responsibilities

The Patient Safety Incident Investigation Lead will provide Trust-wide oversight of incidents requiring full investigation / learning response, plus assuring quality and effectiveness of all types of patient safety investigations/learning responses as part of the Trusts annually agreed Patient Safety Incident Response Plan (PSIRP).

The Patient Safety Incident Investigator will undertake robust patient safety incident investigations/learning responses by working with a range of internal and external stakeholders to investigate, analyse and use credible evidence-based intelligence to agree and present findings using the agreed methods within the Trusts PSIRP.

The outcome will be the identification of systems learning from a patient safety incident/s that will guide sustainable clinical risk reduction and quality improvement of services, ensuring that a restorative, just and learning culture and human factors principles are maintained. The Patient Safety Incident Investigator will provide support to the wider Patient Safety Team/Divisions/teams.
