Paediatric Renal Transplant and Transition Nurse Specialist

We are looking for a motivated, enthusiastic, experienced paediatric nurse to join the specialist children's renal service. The post holder will be working closely with the paediatric renal team to develop and advance paediatric renal nursing practice across various care settings.

Excellent communication and organisational skills are essential to this role. You will need to demonstrate a practical, forward thinking approach to service development and evaluation and the ability to work well within a team.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

The successful candidate will manage a caseload of children and young people from pre-dialysis, transplantation work up, post-transplant, through to transition to adult services. The post holder will work with the child and family, and wider multi-agency team to support young people transitioning to adult services.

The clinical nurse specialist will also cover renal outpatients and haemodialysis depending on service demands, training will be provided. Once trained the right candidate may be expected to support the Haemodialysis on call

Routine responsibilities include promotion, identification, assessment and management of care of potential paediatric transplant patients. To act as a point of contact for families, multidisciplinary team, other hospitals and other agencies in the community for specialist advice and information. To support young people transitioning to adult services, participating in the Transition Youth Program workshops.

Home and school visits are also an integral part of the role, supporting young people transitioning to adult services.
