We are recruiting for a Children's Diabetes Clinical Nurse Specialistwho shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.

This is an ideal opportunity for a qualified ward nurse to work alongside the diabetes community team one day per week to enhance their knowledge and expertise in the management of both Type 1 and Type 2 diabetes for children and young people and their families within the Medway and Swale area. The successful candidate will also become the primary ward-based Link Nurse, acting as a resource and support for both ward staff and the inpatient families with diabetes.

Main duties of the job

The post holder is expected to establish and maintain positive interpersonal relationships with other staff members characterised by trust, mutual respect, and open, honest communication, in line with the Trust BEST values.

These include:
  • Members of the multidisciplinary team including the Medical teams, Allied Health Professionals, Play team, Managers, HR colleagues
  • Broader teams outside of children's services eg Maternity and gynaecology, Children's ED, Theatres and recovery, Imaging, adult wards/services
  • Children's community nursing team, Clinical Nurse Specialists, Safeguarding Children and Vulnerable Adults team, CAMHS, Children's Social Care
  • Children, young people and their families/carers
  • Visitors and members of the public, volunteers, work experience student

Job responsibilities

The Childrens Diabetes Clinical Nurse Specialist is a post with a clear focus on improving the care of Children and Young People (CYP) and to support them and their families to manage their diabetes from diagnosis through the transition to adult services.

In conjunction with other members of the paediatric diabetes multi-disciplinary team, co-ordinate care for children and young people with diabetes and their families, assessing, developing, implementing and evaluating programmes of holistic care and providing specialist clinical advice to health care and others.

Uses specialist clinical judgement and autonomous decision-making concerning diabetes care and management of CYP. Autonomous decision making to include blood glucose data review and titration of insulin doses in accordance with local policy. The post holder will work in collaboration with medical and other multi-disciplinary staff to provide a comprehensive service focused on diabetic children and young people and participate in teaching programmes to parents/carers, children, young people and the direct staff working in nurseries and schools with this group of children.

Person Specification

Qualifications

Essential

  • RSCN or RN (children's qualification) with current registration on the NMC register
  • Willingness to undertake further diabetes courses (i.e. postgraduate diploma encompassing specialist diabetes component)


Knowledge

Essential

  • Knowledge of NMC Code of conduct and application in practice
  • Knowledge of national guidelines, safeguarding policies and procedures.
  • Knowledge of pump usage, blood glucose control including Drug management, Carbohydrate counting and insulin adjustment and equipment for children and young people.


Experience

Essential

  • Knowledge of NMC Code of conduct and application in practice
  • Understanding of and commitment to Equal Opportunities and ability to implement in the job.
  • Demonstrate experience working in a diabetes environment - hospital & community.


Skills

Essential

  • Ability to manage a busy and varied workload and to make informed evidence based decisions under pressure
  • Demonstrate a clear understanding and practical experience of effectively leading and implementing change.
  • Ability to communicate effectively verbally, in writing and electronically.
