An opportunity has arisen for a Physiotherapists to join the Maryport & Cockermouth Rehab Team, located in North Cumbria, next to the beautiful Lake District.

We are a motivated and proactive team, supporting a varied workload. We provide admission prevention through our urgent community response (UCR) service, and supported discharges via the discharge to assess pathway (D2A). As a rehab team we also manage the long term rehab of patients with a wide range of co-morbidities. We assess and treat patients within their home environment, as an out-patient and in clinics setting.

The team consists of OTs, Physios, HCP Co-coordinators, APs, Hub administrators, RAs and Home Care Practitioners (HCP). We work very closely with the District Nurse team, GP practices as well as many other community services and agencies / professionals as part of the MDT.

There is a strong commitment to continuing professional development. This post offers the chance to develop clinical, supervisory and managerial skills so would suit an experienced band 5 wanting to step up to a band 6 level.

The successful candidate will need to be motivated and able to demonstrate excellent assessment and communication skills as well as good team working. Ideally with previous experience working in the community, leading group sessions and managing clients with complex needs.

Main duties of the job

We work closely with GPs, Acute Services, Community hospitals, CHOC, Adult Social Care and third sector organisations. It's key community health focus is to provide care to prevent hospital admissions and to facilitate early discharge. We achieve this through our UCR service and D2A pathway, both of which operate 365 days per year. With ongoing rehab, we aim to enable patients to manage their own conditions and remain in their own homes, enhancing both their independence and quality of life.

The Community Rehab Teams provide assessment and rehabilitation for patients with a wide range of long and short-term conditions. Patientsare seen as in-patients (in Community Hospital beds or Interim Community beds) in their own homes (including residential and nursing homes) or as out-patients (in clinic setting or Rehab Gym)

The core working hours of the service are Mon to Fri 8.00-16.00 but the post holder will be included on a rota to cover late shifts, weekend working and bank holidays. This is to support ICC working 7 days a week, 365 days a year 8am until 8pm with the primary focus of attending UCR visits and completing D2A discharge assessments. The post holder will also be included on the Lead professional of the day rota (LPOD) based in the ICC hub The LPOD role can be busy and fast paced requiring organisational and communication skills with clinical decision making.

Person Specification

Qualifications

Essential

  • CSP recognised Degree/Diploma or equivalent in Physiotherapy
  • HCPC registered
  • Member of the Chartered Society of Physiotherapy


Desirable

  • Evidence of attendance at relevant post graduate courses


Experience

Essential

  • Completion of core junior rotations (Respiratory/neurological/musculo-skeletal/elderly care)
  • Evidence of Continuing Professional Development


Desirable

  • Previous experience of working at a Band 6 level
  • Experience of working within a community setting.
  • Experience of working within a multi-disciplinary team
