This role would be suitable for a qualified non medical Prescriber with experience in managing heart failure or cardiovascular disease. We welcome applications from experienced Nurse Specialists or Advanced Practitioners who have experience in a relevant related field such as nephrology, long term conditions or practice Nursing.

A training post could be considered for applicants without non medical prescribing and specialist experience/ training. This would be paid at a Band 6 whilst appropriate training, competencies and experience are achieved (approximately two years).

More detail bout the role:

The North Devon integrated heart failure service deliver evidence based specialist care across Acute, Outpatient and Community settings. We cover a vast rural area and so driving is essential.

Working closely with the Cardiologists, Physiologists, Arrhythmia and Cardiac rehab teams, the service aims to deliver high quality care and good outcomes for patients with a confirmed diagnosis of heart failure.

This is an exciting time for our service, having recently delivered a remote monitoring pilot which has enabled closer monitoring of patients following hospitalisation and facilitated a means to more timely optimisation of guideline directed disease modifying therapies. It is hoped that we will be able to scale this project, offering rapid optimisation to the majority of our patients using this method.

We are looking for an experienced Nurse with a specialist interest in heart failure to join our cohesive and friendly team. The team are supported in regular attendance at educational events to maintain and develop their specialist skills and knowledge.

Working Pattern: 22.5 hours per week, 9am to 5 pm 3 days (variable) per week
