We are looking for a highly skilled and motivated Advanced Nurse Practitioner to lead the community-based nursing services within the Community Resource Team (Frailty) within Caerphilly.

Working across Primary, Secondary and Community Care, with close alignment to Neighbourhood Care Networks (NCNs), the ANP will be responsible for operationally delivering services across a number of disciplines, managing a large number of nursing and HCSW staff working within a robust clinical and financial framework.

As a clinical expert in the field of care of the elderly and rehabilitation, with a high clinical profile, the priority will be on the provision of safe, effective and efficient care. You will work as an advanced, autonomous practitioner to provide patient-centred care, encompassing expert skills in assessment, examination, identifying differential diagnosis for the treatment of patients utilising our services.

Main duties of the job

The post holder is identified as a clinical expert in the field of care of the elderly and rehabilitation, with a high clinical profile. Priority will be on the provision of safe, effective and efficient care. Post holder is expected to work as a highly skilled senior practitioner with expert knowledge, delivering evidence-based practice to a designated population.

As an advanced, autonomous practitioner will provide patient-centred care, encompassing expert skills in assessment, examination, identifying differential diagnosis for the treatment of patients utilising our services.

The post holder will exercise a high degree of personal professional autonomy and make critical judgements to satisfy the expectations and demands of the role. The post holder will be guided by local and national policies, and identify ways to interpret and advise relevant stakeholders accordingly.

Post holder will be expected to act as a practice leader, managing their own workload and work across professional, organisational, agency and system boundaries to improve services and develop practice. They will lead, support and manage the nurse practitioner team within the community resource team, Redwood Inpatient Unit (12 beds) and will also in reach to care settings where required.

In accordance with the Advanced Clinical Practice framework , will align duties and responsibilities to the 4 pillars of advanced practice.

Also will contribute to the primary and community care Tier 1 on call rota.

Advanced clinical duties outlined below.
  • Demonstrate highly advanced specialist knowledge and expertise across a number of subject areas to include clinical and research developments throughout the care pathway, underpinned by academic and practice evidence.
  • Provide a range of work procedures and practices which require advanced theoretical knowledge and practical experience: clinical advanced examination and treatment options including independent prescribing.
  • Demonstrate the ability to lead and promote advanced knowledge and skills within the multidisciplinary team using best evidence-based practice in all patient interventions.
  • Conduct advanced physical assessments, examinations and investigations to identify highly complex and unstable health needs, and aid diagnoses and develop a comprehensive and appropriate plan of care. Recognise symptoms that would suggest differential diagnosis.
  • Use advanced clinical assessment skills to autonomously manage patients presenting to the nurse led ward or community resource team, to ensure that optimal physical and psychological care needs are met.
  • Use autonomous decision-making including judgements involving complex facts or situations requiring analysis, interpretation and comparison of a range of options in relation to the care and management needs of the patients on a day-to-day basis within the clinical environment.
  • Use of extensive advanced knowledge and expertise across a number of subject areas to include clinical and research developments.
  • Exercise expert advanced clinical judgment when managing and delivering patient care, including challenging situations where there are a number of complicated aspects to consider. Ensure that issues of concern are escalated appropriately.
  • Use best practice, analysis of evidence-based practice and use of benchmarking to inform clinical practice.
  • Be accountable for the direct delivery and implementation of clinical care on a day-to-day basis on the nurse led ward and the community resource team.
  • Provide support and engagement throughout the patient pathway, including end of life, this will include clear methods of supporting patients, families and MDT through difficult and complex situations.
  • Use of advanced clinical skills and autonomous judgement to prescribe drug treatments for patients following interpretation of clinical findings, arranging bloods and other investigations.
  • Make decisions and judgements on a variety of complex facts or competing demands i.e., unplanned emergency and non-emergency situations.
  • Provide detailed and transparent reports of workload, outcomes and recommendations for future developments: demonstrated in advanced practice portfolio.
  • Provide personal care plans, implement and evaluate care, including the independently prescribed medications, monitor therapeutic programmes and provide health education and promotion.
  • Be proactive in developing new ways of working, identify and set up advanced practitioner led initiatives to increase quality of life for patients, limit disease and enable speedier access to services.
  • Provide support and in reach to Caerphilly care home and acute settings when required
  • Provide support and management throughout the patients journey including palliation and end of life.
  • Monitor and review the effectiveness of interventions with the patient and colleagues to meet changing and complex needs and established goals of care.
  • Demonstrate own personal development with self-directed learning and identifying and accessing internal and external development opportunities, as per individual advanced practice portfolio.
  • Evaluate treatments through monitoring and interpretation of clinical indicators, with medication review and/or onward referral as required.
  • Decision making regarding highly complex facts or situations which require the analysis, interpretation and comparison of a range of options.
  • Demonstrate advanced specialist skills and expert advice, providing a detailed treatment rationale, including the balance of risk and benefit with any recommendations.
  • Use of advanced clinical skills and autonomous judgement to prescribe medication in view of interpretation of clinical findings.
