This post is based at Magnolia Inpatient rehabilitation unit, a 34 bedded inpatient rehabilitation ward with 12 beds dedicated to patients with Stroke and neurological diagnoses. As part of a well-established Stroke/ Neurology MDT you will work closely with a team including Physiotherapists, Occupational therapists, Speech and Language therapists, Dietitian, doctors, nurses , support worker & health care staff, social worker and a patient flow co-ordinator. The role is primarily to deliver high standard of specialist Stroke/ neurological physiotherapy input to inpatients admitted to our rehabilitation wards with potential to extend therapy services into the community. You will be a self-directed clinician who works with their patient to ensure the patient reaches their full and optimized potential.

Main duties of the job

Within the rehabilitation service the post holder who by reason of their specialist knowledge, underpinned by theory, practice and experience, will deliver a high quality specialist service, providing expert clinical, education and support to patients and their carers The post holder will provide patient assessment, case coordination, and treatment and planned discharge for adults presenting with Stroke/ neurological presentations.

The post holder will be responsible for the education of, monitoring and delivery of service provided by junior staff and therapy support workers.

Job description

Job responsibilities

Clinical

To be professionally and legally accountable for all aspects of own work, including the management of patients in your care and work delegated to more junior or support staff.

To undertake a comprehensive specialist assessment of complex patients using advanced clinical reasoning skills and assessment techniques to provide an accurate clinical diagnosis of their condition.

To carry out joint assessments with others members of the MDT as appropriate.

Formulate and deliver individual physiotherapy treatment programmes based on an extensive knowledge of evidence based neuro/ Stroke rehabilitation practice and have the expert knowledge and skills to apply a wide range of treatment options. Evaluate patient progress, reassess and modify treatment programmes as required.

Develop comprehensive management plans in conjunction with other members of the multi-disciplinary team. This includes recommendations based on sound clinical judgement, for referral/transfer to other inpatient, outpatient or community services for specialist ongoing management. This will include discussion of patient care, patient progress and prognosis and involvement in ongoing management planning.

Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

Where appropriate to represent the neurology and stroke physiotherapy service, or Trust regarding physiotherapy services.

Person Specification

Ohter requirements

Essential

  • Car owner/ driver with valid UK driving license with access to car for work use.
  • Able to demonstrate a strong commitment to delivering patient focused care.


Qualifications

Essential

  • Recognized Diploma/Degree in Physiotherapy
  • Evidence of Post graduate courses and relevant postgraduate training both theoretical and practical including Stroke & Neurology areas.


Desirable

  • Post-graduation/Msc in relevant area (elderly care /neurology)
  • Membership of professional body or CIG
  • Leadership training


Skills and Abilities

Essential

  • Experience at band 7 level or significant experience at band 6 level/equivalent in adult neurology and / or Stroke.
  • Ability to process, manage and communicate complex information.
  • Advanced clinical reasoning and problem solving skills within stroke care and other relevant clinical areas.
  • Ability to prioritise own and others workload within a busy environment.
  • Audit/evaluation skills, supervision & teaching skills


Desirable

  • Good presentation skills.


Experience and knowledge

Essential

  • Experience at band 7 level or significant experience at band 6 level/equivalent in adult neurology and / or Stroke
  • Demonstrate appropriate postgraduate experience working across physiotherapy core areas to support the rehabilitation needs of the patient.
  • Experience of working as part of MDT team in the NHS, goal planning and developing care plan.
  • An advanced breadth of knowledge about Neurorehab/ stroke care and evidence based/best physiotherapy
  • A good working knowledge of the NHS NHS Long term plan, relevant national guidelines,


Desirable

  • Previous experience at band 7 level in neurology/stroke.
  • Experience of 7 day service delivery
