An exciting opportunity has arisen for two Specialist Mental Health Practitioners to join the Hiraeth (Complex Needs) Service on a permanent basis. The Hiraeth team provides trauma and relationally-informed care to people who have complex and multi-layered needs and provides such care as close to home as possible.
One of the team's recent developments has included the development of the Acorn project, delivered in partnership with a third sector organisation. This project is designed to transform the way in which we transition young people from children's services to adult services. As such, candidates would benefit from having worked across the lifespan.
Your role will be to facilitate and provide evidence-informed and high quality care to our service users this may be through care coordination, intensive case management or through specialist intervention.

- Provide mental health assessments and offer biopsychosocial and evidence-based interventions where appropriate
- Provide specialist advice and support regarding patient management. This includes assisting with referral pathways and the provision of consultation, supervision and training to staff from a range of disciplines to improve mental health care of the local population within community and inpatient services.
- To develop and enhance communication between patients, Primary Care, Secondary Care and local community resources including those provided by the Third sector, thereby ensuring a good patient journey.
- To lead in evaluation, audit, learning, training and development within the role, including producing evidence to inform future service design.
