The Royal Marsden NHS Foundation Trust has a unique history of leading the way in the development of new approaches to cancer care. The Palliative care team continues this tradition. The fixed term role, is a unique opportunity to deliver Palliative Care service improvements to the Royal Marsden NHS Foundation Trust. This service has been designed to promote early referral to palliative care, advance care planning and referral to specialist palliative care through the use of a set of triggers in site specific tumour groups in the out-patient department. The post holder will be responsible for educating the primary treating teams in the use of the triggers and subsequent collation of data to report on key outcome measures. The postholder will take responsibility for the co-ordination and management of the project alongside the Consultants in Palliative Medicine. We are now looking to recruit a C Nurse Specialist to work as part of this multi-professional team. In the first instance, you will need to be a registered nurse with demonstrable post registration experience in cancer/palliative care. Educated to at least degree level, you will also need to be confident in delivering education and training to all staff groups, having attained some form of post registration qualification in this. This post will offer this service to patients at both the Chelsea and Sutton sites of the hospital. We would welcome and encourage an informal visit.

Main duties of the job

- The post holder will be responsible for supporting oncology team colleagues in the use of the triggers tool and subsequent collation of data to report on key outcome measures. The postholder will take responsibility for the co- ordination and management of the service alongside the Consultants in Palliative Medicine.
- The post holder will be part of the wider Symptom Control & Palliative CareTeam.
- To assist with the organisation and implementation of the Triggers service and liaising closely with the multi-disciplinary team to facilitate a co- ordinated approach to patient care.

- Where appropriate to support patients with symptom control and involve specialist palliative care team as needed.
- To assist the medical team in the assessment of patients/volunteers and monitoring their condition as appropriate.
- To establish and maintain good working relationships with supporting clinical services and primary teams.
- To work effectively as part of the multidisciplinary team and to contribute to the ongoing development of the clinical unit by acting as a role model for staff in areas related to Palliative Care
