An exciting opportunity has arisen to join Lancashire and South Cumbria Foundation Trust as a Band 6 Occupational Therapist within the Community Rehabilitation Team.
The team includes the Falls Service, Community Physiotherapy Service, Intermediate Care Service and Rapid Assessment service.
We are a team of Occupational Therapists and Physiotherapists working closely with the multidisciplinary team to prevent further deterioration in patients' physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.
We are looking to appoint a dynamic, enthusiastic and forward thinking Occupational Therapist to join our service; with excellent communication skills, who can demonstrate sound clinical assessment skills and who has the ability to work autonomously as well as part of a team.
The role involves assessing patients within either an intermediate care setting, or in patient's own homes. There may be a requirement to assess and treat patients within a ward setting on occasions. There may be times when flexible working across other community teams such as Home First and SPoA (Single Point of Access) may be required

Main duties of the job
Provide a high standard of specialist Occupational Therapy input to the designated therapy team. Hours may be required to be worked over 7 days between 08.00am and 08.00pm, depending on service need.
To assess and treat patients within a ward or community setting as an autonomous practitioner
Occupational Therapy assessment of patients with diverse presentations and complex physical and psychological conditions, to contribute to diagnosis and develop and deliver an individualised treatment programme.
Hold responsibility for own complex caseload and be responsible for a defined area of the service or a particular patient type, working without direct supervision.
The postholder will ensure their own personal development and participate in the development of others
To supervise, educate and assess the performance of Occupational Therapy students to ensure the standard of practice and teaching meets the degree standard
Undertake evidence-based audit and contribute to research projects. Make recommendations to clinical lead/manager for service improvement
