Norfolk Community Health and Care NHS Trust is pleased to announce that an opportunity has arisen for an enthusiastic and motivated Physiotherapist to join our community teams. We have 1 post in NN3 Aylsham Team and 1 post in our NN1 Fakenham/Holt Team. Opportunity may also arise to help support our community in-patient team at Kelling Hospital. This can be a band 6 post or can be a linked grade providing you with the opportunity to progress from a Band 5 to a Band 6 in line with an agreed set of competencies.We are looking for someone who has completed 2-3 rotations or equivalent at Band 5 level.You will be supported in gaining skills in advanced clinical reasoning and problem solving as well as skills in complex assessment and intervention. Excellent interpersonal skills and the ability to discuss assessment and interventions empathetically with service users, relatives and carers is critical to this role, Support is available from Clinical Leads and Community Operations Managers. Band 6 competencies are expected to be reached within 12-18 months of appointment.

Main duties of the job

You will assess and prescribe exercise programmes to patients with, but not exclusive to, frail ad elderly, palliative care and rehabilitation needs, to achieve quality and independence where possible.

You work with the integrated team to facilitate early discharge from hospital, and also to prevent unnecessary admission to hospital.

You will be working with health care professions , statutory and non-statutory agencies to provide a seamless integrated service to our service users.

You will provide advise, support and supervision to junior staff, therapy assistants and students.

You will deliver high quality physiotherapeutic interventions and health care to patients within a variety of community settings, which may include in-reaching on to the in-patient units as needed. To provide proactive, holistic case management and rehabilitation whilst working with patients to help them set realistic goals. Utilising health coaching skills to actively engage with patient and their family to ensure that expectations are appropriately managed.

About us

You will work with a small team of friendly, approachable physiotherapists, occupational therapists, therapy assistant practitioners and community nurses. The posts are based at Kelling Hospital Site, Fakenham Medical Practice and Aylsham Health Centre respectively. You will be travelling out to see patients in their own homes or in care homes in Fakenham, Wells, Holt and Sheringham and surrounding villages within NN1 whilst the NN3 post covers Aylsham, Drayton, Taverham, Coltishall and Horsford. You may also be required to provide cover on the 24 bed in-patient unit at Kelling Hospital if needed.

Person Specification Qualifications Essential
  • Diploma or first level degree in Physiotherapy
  • Current HCPC registration
  • Clinical Educator's course or equivalent
Experience Essential
  • Evidence of working as band 5 physiotherapist across a broad range of specialities eg Orthopaedics, MSK, Neurology, Care of the Elderly etc in acute and community settings
  • Experience of performing falls assessments and gait analysis
  • Experience of working with/as part of multidisciplinary teams and partner agencies
  • Experience of working with patients with multiple pathologies and managing holistic patient centred care
Skills, Abilities and Knowledge Essential
  • Ability to organise and prioritise workload independently and support others to do so
  • Evidence of understanding of Safeguarding issues relating to the care of vulnerable adults and older people
  • Ability to work on own initiative
  • Evidence of attendance on short courses and/or in house training related to physiotherapy post-registration
  • Good computer skills including SystmOne and ability to work on information management systems
  • Relevant safeguarding training
  • Working knowledge of SystmOne
  • Experience of audit/research

  • Evidence of excellent verbal, written and interpersonal skills appropriate to supporting other members of the team

Other Essential
  • Post holder will be required to work standard hours between the hours of 9-5 Monday to Friday, and will need to be flexible in extenuating circumstances when there may be a need to be flexible with working hours
  • Must hold a full and valid driving licence and have access to a vehicle for work
  • The post holder will need to be passed as fit to perform the full duties of the post by Occupational Health , with any reasonable aids and adaptations as appropriate
