This project, which is a community-based intervention, aims to improve outcomes for children and young people with asthma, including reduced asthma admissions and ED attendances. Improved outcomes also include less school absences due to asthma related illness, increased staff knowledge, confidence in relation to asthma management and increased ability to support the CYP with managing their asthma in school.

Effective asthma management at school has many positive outcomes including improving the ability to learn and participate fully during their school day. Promoting an asthma friendly environment through training and the adoption of asthma focused policies and procedures will ensure that CYP are safe while they are in school.

The post is hosted and supported by the secondary care acute provider trust but the focus of the service will be to implement the asthma friendly schools programme across York education settings and upskill and empower Primary Care colleagues to provide asthma care to CYP and their families.

The Asthma Friendly Schools Programme is a national initiative to promote the health & wellbeing of pupils with asthma. This ensures they have minimal disruption to their education, can access the full curriculum and are not disadvantaged by their disease.

If you are currently employed by the Trust and you wish to apply for this role on a secondment basis, please seek permission from your current line manager before applying for the post.

Main duties of the job

  • Offering asthma friendly school (AFS) initiative to all York schools and working with them collaboratively to embed AFS standards across all schools in York.
  • The AFS project nurse will work directly with CYP, primary care, secondary or tertiary care (where CYP is under specialist care) and schools to ensure each CYP has a Personalised Asthma Action Plan (PAAP) and a copy of this is held by schools.
  • Maintaining high standards of record keeping in line with York and Scarborough Teaching Hospital NHS Foundation Trust and the Nursing and Midwifery Council standards, including regular audit of standards.
  • Ensuring correct information governance procedures are followed including appropriate consent from parents & young people are obtained and recorded in line with legal requirements
  • Ensuring clinical & safeguarding supervision processes are embedded within the service governance arrangements
  • Delivering training, advice and support to parents and carers in a variety of settings using an evidenced based and robust competency framework
