Specialist Learning Disabilities DirectorateSpeech and Language TherapistTelford and Shropshire Community Learning Disability TeamWe are seeking a part time, highly motivated Band 6 Speech and Language Therapist to work within the Community Learning Disability Team in Shropshire where we provide high quality, community based, integrated services for adults with learning disabilities.This is currently a 9-month fixed term opportunity to cover Maternity Leave. If successful, you will work independently under the leadership of the Professional and Team Lead, whilst having the opportunity to develop yourself personally and professionally during this time. A valid UK driving licence plus the use of your own car for business purposes is essential due to the large rural nature of our catchment area.

Main duties of the job

To provide specialist Speech and Language Therapy assessment, diagnosis and treatment programmes to individual adult clients with complex learning and physical health needs referred to CLDT in Shropshire. To be an autonomous practitioner, holding a small mixed clinical caseload of those with swallowing and communication needs working without direct supervision within the context of a multi-disciplinary team.To support less experienced staff and students and colleagues deliver the Trust contracted services.To provide expert advice in relation to swallowing and communication. Signposting and providing guidance and information to health and social care professionals, clients, carers and relatives.

Job responsibilities

1. Be professionally accountable for all aspects of own work, including caseload management and direct clinical care. To work within the Royal College of Speech and Language Therapists and Health and Care Professionals Councils code of conduct and scope of practise.2. Obtain and record informed consent from clients at each contact in accordance with Trust policy.3. Carry out comprehensive specialist Speech and Language Therapy assessments for clients with complex learning and physical health needs. Interpret clinical information and data related to clinical parameters along with non-clinical information and use clinical reasoning skills to accurately diagnose and develop specific treatment plans.4. Provide specialist advice to other professionals or single assessments to meet clients holistic care needs.5. Formulate and deliver individual treatment plans based on specialist knowledge, evidence-based practise, national and local guidelines using profession-specific clinical skills and techniques. (E.g. 1:1 interventions, carer education, patient education).6. To undertake highly complex swallowing and communication assessments.7. Maintain up to date knowledge of Speech and Language Therapy assessment and intervention evidence base developments for this client group.8. Maintain up to date knowledge of instrumental assessment available and to be able to signpost to services who assess and provide this.

Person Specification

Qualifications & Training

Essential

  • Degree in Speech and Language Therapy.
  • Unrestricted registration with Health and Care Professions Council (HCPC).


Experience

Essential

  • Competence with managing communication needs of clients with complex learning and physical health needs.


Desirable

  • Worked as part of multidisciplinary team.


Skills, Knowledge & Abilities

Essential

  • Demonstrated knowledge and skills in competencies associate with working with people with complex learning and health needs.
  • Excellent interpersonal and communication skills.
