Part time post for Clinical Nurse Specialist for adults with Xeroderma Pigmentosum (XP)

Guy's and St Thomas' NHS Foundation Trust, is a leading centre for patients with complex multisystem diseases. The new and specially designed Rare Disease Centre, at St Thomas Hospital, hosts the Xeroderma Pigmentosum (XP) Clinic for the St John's Institute of Dermatology, one of the world's leading centres for skin disease.

Xeroderma Pigmentosum (XP) is a severe inherited disease, causing skin cancers, eye damage and sometimes neurological degeneration.

Along with learning more about this rare condition, the aim of the National XP service is to share its unique expertise for the long-term benefit of all those affected by XP worldwide.

This Post will provide an ideal opportunity for a highly experienced, motivated nurse to consolidate clinical, research, education and leadership skills whilst facilitating effective care and support to this patient group. The role will involve a mix of outpatient clinics at Guys and St Thomas' hospital sites, travel around the whole UK to patients' homes and universities and places of work.

You should possess excellent communication skills for good inter-professional communication and a positive team working environment.

Specialist experience is not essential, but being interested in developing knowledge in dermatology, minor skin surgery, neurology, ophthalmology or genetics is key. The post includes regular travelling.

Main duties of the job

• Home visits to complex patients (across UK) - all newly diagnosed patients automatically receive a home visit. All existing patients offered an annual home. Other visits indicated by clinical need.
• Give diagnoses to new patients and support during life changing impact to patient and whole family.
• School / University / work place visits
• Travelling: Due to distance, some home visits require overnight stay
• Telephone/ email support and advice to patients
• Telephone/ email support and advice to health care professionals in UK and internationally
• Co-ordinating with local teams for tests and surgery
• Patient related admin
• Social services / continuing health care meetings
• MDT prep and follow up
• Organising skin cancer surgery within the trust and at a local level
• Surgery /Histopathology MDT, tracking all surgery outcomes
• Document , monitor and feed back histology results and maintain lesion monitoring data base.
• Inpatient support (ad hoc as required when XP patient admitted) within the trust or nationally
• Working as part of the MDT in XP clinics
• Skin Checks
• Assisting in Skin Biopses
• Dealing with Emergency adhoc Skin Lesions

Person Specification

Experience

Essential

• Extensive experience of working autonomously at a level within the specialist area delivering effective patient focused care
• Proven evidence of management and leadership skills
• Previous experience of student supervision and clinical education at undergraduate level
• Experience of delivering patient advocacy
• Experience in audit, research, and evidence based care-essential
• Experience in delivering change management both personally and as a facilitator

Qualifications

Essential

• BSc in nursing or equivalent qualification/training
• RN1 Registered Nurse on the NMC register. To hold the appropriate level for the area of speciality.
• post registration qualification in appropriate area of speciality or equivalent post qualification experience

Desirable

• To have or working towards a non medical physical assessment course
• To have or working towards non- medical prescriber qualification
