The Patient Safety Team at UHD are looking to recruit two Patient Safety Facilitators to drive our safety culture and support implementation of our new Patient Safety Incident Response Plan.

This is an exciting opportunity to be at the forefront of organisational change and help drive an open reporting, learning and improvement culture for patient safety across the Trust and within the Dorset system.

The post-holders will work closely with all staff across the Trust to support and facilitate the process of patient safety incident reporting, promoting learning following patient safety events and support changes inpractices to prevent recurrences in line with Trust Policies.

Locations: Royal Bournemouth and Poole Hospital. This post will require cross site working

Main duties of the job

  • Support the implementation of the new patient safety incident response framework (PSIRF) and associated plan (PSIRP)o Promote the trust culture of open and honest reporting of a patient safety incident that may impact on the quality of patient care in accordance with trust policies on patient safety incident reporting and risk managemento Oversee patient safety activity within the organisation and support care groups to meet the needs of the PSIRF process through coordinating and be an effective leader with the patient safety and quality agenda within care group rapid review meetings and the organisations PSIRF insight and oversight meetings.o Provide guidance and support to care groups in responding to incidents, ensuring that best practice standards for incident investigation, record keeping and learning are maintained at all times.o Ensure the principles of engaging with people affected by incidents are followed in line with the PSIRF standards.o Monitor safety actions and improvement plans arising from patient safety activityo Deliver staff training related to patient safety activityo Collate and analyse information and provide detailed reports on behalf of the patient safety team for care group rapid review meetings and PSIRF insight and oversight meetings,

Person Specification



  • Educated to Degree level or Equivalent Clinical Qualification


  • Management/ Leadership Qualification
  • Diploma in Risk Management or similar NEBOSH Certificate



  • Three years' experience in a clinical/management role within the NHS with direct experience of managing patient safety, risk and/or quality governance.
  • Experience in incident investigation and the use of data analysis with complex analytical skills.
  • Experience of patient engagement including providing sensitive or distressing information and dealing with difficult situations including the resolution of conflict.
  • Experience in training/presenting to all levels of staff on risk, patient safety and/or quality governance related subjects
  • Experience of Risk Management systems (e.g. Datix)



  • High level of communication skills with well- developed interpersonal skills necessary to establish and maintain effective working relationships amongst a multidisciplinary group, including members of the Executive team and other senior leaders.
  • Ability to lead and facilitate change whilst engaging and motivating others
  • Excellent verbal, written and problem-solving skills including report writing.
  • Demonstrable ability to manage frequently changing and competing priorities
  • Excellent understanding of the current incident reporting frameworks and methodologies and the new Patient Safety Incident Reporting Framework and National patient safety strategy


  • Use of QI methodology to improve practice/services.
  • Experience of liaising with external stakeholders
