Southern Health NHS Foundation Trust are excited to be recruiting a Treatment Escalation Plans Programme Manager to work with Clinical Directors, clinicians, operations managers and patient representatives across Hampshire to deliver an initial solution, develop a staged future roadmap and lead programmes of work to achieve the aim of enabling visibility of Treatment Escalation Plans across all providers within respective IT systems in order that we are all aware of patients wishes and requirements and can act accordingly. Working with partners that include but are not limited to South Central Ambulance Service, the soon to be formed Hampshire and Isle of White Healthcare NHS Foundation Trust and Acute and Primary Care providers across the Hampshire region this role supports the delivery of a high-profile project, driving the delivery of improvements in clinical management, ensuring patient wishes and preferences are visible and acted upon. This role will also ensure that clinicians are provided with the tools to support the National drive to care plan earlier in patient care pathways.

Main duties of the job

The aim of this project is to work with Clinical Directors, clinicians, operations managers and patient representatives across Hampshire Place to deliver an initial solution, develop a staged future roadmap and lead programmes of work to achieve the aim of enabling visibility of Treatment Escalation Plans across all providers within respective IT systems in order that we are all aware of patients wishes and requirements and can act accordingly.

This includes but is not limited to South Central Ambulance Service, Hampshire and Isle of White Healthcare NHS Foundation Trust and Acute and Primary Care providers across the Hampshire region.
