A fantastic opportunity has arisen to join the Diabetes Specialist Nurses and Cystic Fibrosis (CF) team in an exciting new band 7 dual role.

At Frimley Park Hospital, we host a specialist adult CF unit, recognised by the CF Trust. We have a 5 bedded inpatient ward and 5 dedicated outpatient clinic rooms. We currently care for approximately 165 patients, mainly from the Greater London, Surrey, Hampshire and Berkshire areas. We have been fortunate to secure funding for additional specialist diabetes nurse time to support our expanding group of patients with CF Diabetes. This is an exciting and evolving area of diabetes, and we are an innovative team who aim to optimise outcomes and stay abreast of the latest developments, treatment options and technologies. Our comprehensive specialist multidisciplinary team comprises of CF specialist nurses, consultants, dietitians, physiotherapists, exercise practitioners, pharmacists and psychologists.

The post will include 0.56 WTE of CF time to manage and lead diabetes care in people with CF, providing high quality, evidence based, individualised care and treatment in the inpatient, outpatient and community settings. The post holder will provide specialist advice, support, and education to patients, carers and other healthcare professionals.

Main duties of the job

Main duties, tasks & skills required
  • To lead, develop and provide a specialist diabetes nursing service for adults with cystic fibrosis diabetes (CFD). The service covers inpatients, outpatients and community care.
  • To jointly lead the continuous glucose monitoring (CGM) service with the CF dietitians and identify patients requiring CGM.
  • To assess, plan and implement specialist diabetes therapy to meet patients' clinical need in conjunction with the CF team and diabetes consultant.
  • To act as a key contact in providing specialist CF diabetes advice and education for patients, families, carers and support healthcare professionals at Frimley Park Hospital Adult CF centre.
  • To initiate and undertake research and clinical audit.
  • To contribute towards achieving diabetes NSF and CF Trust standards.
  • To have an in-depth knowledge of all statutory guidance.
  • Within general diabetes care, to work within a multidisciplinary team to provide a specialist diabetes nursing service for adults with diabetes, delivering treatment, education and support for people with diabetes and their families. This is covered in both the hospital and outpatient community clinics as follow on care.
  • To be responsible for maintaining and enhancing professional standards of diabetes care.
  • To act as a specialist diabetes resource for medical and nursing staff and allied health care professionals.

Person Specification

Qualifications

Essential

  • RGN level 1
  • Current NMC registration
  • ENB 998 teaching and assessing course or equivalent mentorship preparation course e.g., C & G 730.
  • To have completed a recognised diabetes course
  • Nurse prescribing course (NMP)


Desirable

  • Possess a relevant Nursing Degree or be willing to work towards one.
  • Desire to develop diabetes and management skills.
  • Experience with technologies within diabetes.


Experience

Essential

  • Previous post registration NHS experience in a diabetes environment for at least 2 years.
  • Teaching and supervising of junior staff and students.
  • Effective working within multi-disciplinary teams.
  • Working with IT systems e.g., patient centre.


Skills & Knowlege

Essential

  • Excellent interpersonal skills with the ability to manage difficult and stressful situations effectively.
  • Excellent time management skills
  • Able to prioritise and manage caseload.
  • The ability to work independently to an approved protocol whilst acknowledging limitations and liaising with to Senior DSN or Diabetologist where appropriate.
  • Supervising and teaching fellow colleagues and students.
  • Effective resource management.
  • IT skills


Desirable

  • Experience with CF / CF diabetes patients
  • Experience of audit/research Publication
